TASK ORDER NO. 3 TO
MASTER SERVICES AGREEMENT R-50(F)
BETWEEN
OWNER AND ENGINEER
FOR
MASTER SERVICES
(NON-HAZARDOUS WASTE SERVICES)
This Is Task Order No.3 attached to and made part of the Master Services Agreement R-W(F7 dated
April 4, 2016 between Stantec Consulting Services, Inc., 5 Burlington Woods, Burlington, MA 01803
(ENGINEER),and the City of Salem(City),93 Washington Street,Salem,MA 01970(OWNER). This Task
Order No.3 describes the Scope of Services,Time Schedule,Charges.and Payment Conditions for
the Task Order known as: Limited Traffic Peer Review 14 Bertuccio Avenue Subdivision. Materials
received to date from the City of Salem include the following:
• Definitive Subdivision Pian Set (14 Sheets), dated March 17, 2016 and supporting
documentation as prepared by TTI Environmental, Inc.
• Environmental&Community Impact Statement,dated March 2016 as prepared by The
TTI Environmental, Inc.
PROJECT OBJECTIVE:
The purpose of Task Order No. 3 Is to provide technical engineering support services to the City of
Salem Planning Board in Its review of the 14 Bertucclo Avenue Residential Site Plan. The preliminary
submitted material did not include a traffic Impact study to identify Impacts of the project. As
discussed with City officials, our peer review will be limited to the site circulation/access issues
relative to the safety of vehicular traffic, given site distance concerns at the entry along with the
numerous waivers requested to accommodate the entrance into the subdivision. The developer
has requested three zoning variances for the following three issues.
1. Section IV, A, 1, f:
a. Minimum roadway centerline radius shall be 230'.
i. The Proposed centerline radii are 126' and 134' respectively.
2. Section IV Right-of-Way Improvements Policy:
a. Sidewalks and grassed strips shall be installed on both sides of the roadway for
subdivisions with 3 to 6 lots.
i. Sidewalks and grass strips are only proposed on one side of the roadway at the
entrance into the subdivision.
b. Street trees 3 'h In.caliper; 30 ft. on center are required for subdivisions with 3 to 6
lots.
1. No street trees are proposed at the entrance into the subdivision.
3. Section IV,A. 2,a:
a) Minimum Sidewalk width shall be 5'.
I. The proposed sidewalk is 4' wide.
b) 'In instances where existing street widths are greater than 36 feet the existing j
width shall be met.
1. The existing Bertucclo Avenue right of way width varies, measuring around 40
feet in width.The proposed roadway width at the entry is 39 feet; the
remaining width of the road extension is 40 feet.

4. Section IV, A,4,a:
a) Dead end streets shall not be longer than 500 feet.
i. The proposed dead end street is over 950 feet long, as measured from the
Bertuccio Avenue and Francis Road intersection to the end of the cul-de-sac.
SCOPE OF SERVICES:
Task 3.1 - Limited Traffic Peer Review and Technical Memorandum
Stantec will conduct a site visit to review existing conditions,including roadway geometry and sight
line distances. We will review the site plan submission to identify any site circulation issues,as well as
reviewing the on-site signage and striping. Stantec will focus our peer review on the following
specific concerns identified by the City of Salem.
• Roadway width of 39 feet(narrower than the existing road)
• No trees/vegetation in the entrance
• Radius of curvature less than 230 feet.
• Grade of the proposed roadway off of Bertuccio Avenue
• The proposed sight line distances.
After reviewing materials submitted by the applicant, Stantec will prepare a technical
memorandum summarizing our findings and recommendations for the City of Salem.
Task 3.2- Attend up to one meeting to discuss our technical memorandum.
Stantec will prepare for and attend one public hearing to present our findings and
recommendations to the Planning Board,
PERIOD OF SERVICE:
Work on Task Order No.3 shall commence immediately upon receipt of a Notice to Proceed(NTP)
via e-mail or letter from the City of Salem and conclude with a presentation of our findings at a
Planning Board meeting as determined by the City of Salem.We anticipate submitting our technical
memorandum within 5 calendar days of notice to proceed.
COMPENSATION AND INVOICING:
In accordance with the Period of Service described above our estimated level of effort for this Task
Order No. 3 shall be 23 hours of traffic engineering services at Contract R-50(F)rates plus direct
expenses for travel, etc. as follows:

Unless additional scope Items are added by the City,we propose a total Not-to-Exceed Budget for
Task Order No.3 In the amount of$3,454 consisting of$3,354 in labor and$100 for direct expenses.
TERMS AND CONDITIONS:
The terms and conditions of the Agreement referred to above shall apply to this Task Order No. 3
except to the extent expressly modified herein. In the event of any such modification, the
modification shall be set forth below and the Article of the Agreement to be modified shall be
specifically referenced.
TERMS OR PROVISIONS IN CONFLICT:
If the provisions set forth in the Agreement are in conflict with the provisions set forth in this Task Order
No. 3, the provisions of this Task Order No. 3 shall govern.
Acceptance of the terms of this Task Order No. 3 is acknowledged by the following authorized
signatures of the parties to Master Services Agreement R-50(F):
