The attached daily excavation summary sheet indicates the approximate location of construction activities observed today. The earthwork contractor, WAM General contractors, began placing on-site fill material on the exterior of the building (both for landscape areas and under planned sidewalk areas). The fill was compacted using multiple passes of a Bomag BMP 8500 vibratory roller. The compacted fill material was observed to be unstable and oversaturated, even when allowed time to dry. The contractor imported dry, dense grade fill which was mixed in a 2:1 ratio with the on-site fill for areas under the planned sidewalk location. A 1:1 ratio was initially attempted, but yielded an unstable surface after compaction. Un-mixed on-site fill was utilized for backfill in future landscaped areas. The earthwork contractor, WAM General contractors, placed 6 inches of 3/4-inch crushed stone over the areas inside of the proposed building that were observed to be below the groundwater level. The crushed stone was compacted using multiple passes of a vibratory plate compactor. The contractor then excavated 1-foot of the existing fill within the center of the entire interior of the building. Following excavation, the exposed fill subgrade was compacted using a Bomag BMP 8500 vibratory roller. An additional 1-foot lift of screened, on-site fill material was subsequently placed and compacted using the same roller. The material was found to be firm and stable. The contractor placed frost blankets over the compacted material before leaving for the day. The attached daily excavation summary sheet indicates the approximate location of construction activities observed today. The earthwork contractor, WAM General contractors, continued placing on-site fill material on the exterior of the building (both for landscape areas and under planned sidewalk areas). The fill was compacted using multiple passes of a Bomag BMP 8500 vibratory roller. The contractor continued to import dense grade fill which was mixed in a 2:1 ratio with the on- site fill for areas under the planned sidewalk location. Un-mixed on-site fill was used in landscape areas. The interior of the building was backfilled using imported dense grade placed in one-foot lifts. The compacted material was observed to be dry firm and stable. The purpose of the helical piles is to provide foundation support for the proposed one-story addition to the 83 North Street building located in Salem, Massachusetts. As part of the proposed construction, a previous site building located to the west of the 83 North Street building was demolished and a new addition is planned to be constructed which will abut the 83 North Street building in the approximate area of the demolished building. Each installed helical pile consisted of a 7-foot long lead section containing 3 helical plates of 10, 12 and 14 inches in diameter and 7-foot and 5-foot extensions consisting of 2.875- inch diameter round steel shafts. The helical piles were installed to minimum final installation torques ranging from 3,900 to 5,500 foot-pounds within the natural marine clay deposit, at approximate depths ranging from 46.5 to 58 feet below the ground surface. Based on our observations on-site, the twelve (12) above-noted helical piles, No. A-1 through No. B-6, were installed in general accordance with the requirements of the Contract Documents. Please find enclosed our helical pile location plan and our helical pile installation logs attached to this letter. In addition, a representative from McPhail Associates, LLC (McPhail) was present on-site on a part time basis to observe proofcompaction of the proposed slab subgrade as well as the placement and compaction of fill materials within and outside of the proposed building footprint. The observations of McPhail's field engineer are documented on the attached daily field reports. In summary, slab subgrade preparation was performed in accordance with the recommendations contained in McPhail's letter entitled "Foundation Design Recommendations" dated June 27, 2018. Furthermore, backfilling activities observed outside of the building were performed in accordance with standard industry practice.
