Subject: Submission of "Permit Set" of Drawings

Renovations to No. 5 School Street, Salem, MA
Dear Bill:
This letter, written on behalf of my client, Jessica B. Herbert, is intended to
be delivered at our meeting in your offices tomorrow morning. It concerns
the building permit application for the above-noted project which is now
pending before your department.
Enclosures
Enclosed please find three (3) bound sets of blackline prints of our "Permit
Set" of architectural and structural drawings for the project, dated 22 June
1987, consisting of nineteen (19) sheets. The drawings, which are fully
enumerated in a list of drawings on the title sheet, include the following:
TI, SDI, LI-2, RI-4, Al-9 and SI-2.
Previous Submissions
This submission supercedes an earlier one made by my client to Captain
Turner during the week of 18 May 1987. Those drawings, entitled "Pricing
Set" and dated 20 April 1987, are now moot for permitting purposes.
This submission is fully consistent, however, with all previous filings we
have made with the Planning Board and Board of Health regarding this
project.

Revisions
The "Permit Set" is intended to contain all essential information requirekpfor
your department to review the project and approve it for the issuance of a
general building permit. Towards that end, this set incorporates your
suggestions and requests made at my recent meeting in your offices, on 4
June 1987, with Jim Santo and Jessica Herbert also attending.
Schedule
As you are aware, the work authorized under the demolition permit you
have already granted for this project is substantially complete. In order for
the project to move forward in a timely manner, it is essential that a general
permit be granted at your earliest convenience. We respectfully request that
you do do, with the understanding that any technical amendments or
revisions which you may require will be promptly submitted to your
attention.
Construction Control Procedures
For the record, we acknowledge your determination that this project is
subject to the provisions of the Massachusetts State Building Code regarding
Construction Control (Section 127). Accordingly, we will commence
submitting bi-weekly reports upon issuance of the general building permit.
Such reports will be forthcoming from me, as Architect of Record, or from
Colin G. Simson. P.E., the Structural Engineer of Record.
Thank ,you for your attention and assistance on this project. We look forward
to seeing it sucessfully through the construction phase with you.
