Pre-Construction Review of Entrance Corridor Project

At a regular meeting of the Design Review Board (DRB), upon a motion duly made and seconded, it was
unanimously voted to recommend design approval to the City Planner of the pre-construction documents of the
A.L. Prime gas/service station and associated site improvements.

Design Recommendations
Overall, the DRB found that the proposed changes to the project are minimal and align with guidance from the
Planning Board and MA DOT. The proposed changes also account for architectural revisions, which were
conducted by Derby Square Architects.
The Board recognized that the changes were captured in a clear and comprehensive way, with red markings on
the revised plans, a narrative and list of changes, and a presentation of the revised plans side by side with the
previously reviewed plans.
Upon discussion and deliberation, the Board offers the following recommendations for the City Planner’s
consideration:
•Minimal Changes: The Board recognized that the proposed changes from their previous review are
minimal and align with direction from the Planning Board and MA DOT. Additionally, the changes
encompass refinements from an architect.
•Architectural Revisions: The architectural revisions substituted previous plans and offer more detail. The
substitutions are outlined in the narrative and list of plan changes.

This narrative and list of changes is submitted to the DRB for consideration in their
determination of whether the plans submitted for permitting are consistent with the plans
as previously approved by the DRB.
A set of the plans submitted for permitting has been also submitted with red markup
highlighting all of the specific plan revisions. This document provides a listing of those
changes. All sheets in the permit set are commented on below.
General – The general site layout, building footprint and elevations, driveway locations,
grading, lighting, parking, and signage have not changed. New plan sheets have been
added or substituted to provide architect and structural engineer stamped plans detailing
the building and MA DOT plans required for the permitting of the abandoned utility
disconnects and new driveway on Highland Avenue.
C-0 Cover –
- Updated the index to include new or substituted sheets.

GENERAL NOTES
1. THE GENERAL PURPOSE OF THIS WORK IS TO REMOVE EXSTG IMPROVEMENTS IN
PREPARATION FOR THE CONSTRUCTION OF A NEW GAS STATION AND CONV
STORE
2. THE PROSPECTIVE PROPERTY OWNER WILL OBTAIN ANY APPLICABLE BOARD
AND STATE APPROVALS AND GENERAL BLDG PERMIT. THE CONTRACTOR WILL BE
RESPONSIBLE FOR PERMITS FROM THE LOCAL FIRE MARSHAL'S OFFICE AND
TRADE OR BLDG PERMITS FROM THE LOCAL BLDG DEPT AND ALL ASSOCIATED
TESTING AND INSPECTION REQUIREMENTS.
3. THE CONTRACTOR WILL OBTAIN DEMOLITION PERMIT PRIOR TO ANY WORK ON
SITE, INCLUDING VERIFYING UTILITY SHUTOFFS OR DISCONNECTS, AND ANY

ON-SITE IRRIGATION WELL NOT CONNECTED TO THE DOMESTIC SERVICE.
3. PEAK FLOW BASED ON SIMULTANEOUS OPERATION OF ALL INSTALLED FIXTURES
(TWO RESTROOMS, PLUS THREE SINKS, PLUS HOSE BIB) = 52 GAL/MIN PEAK
FLOW.
4. PRIOR TO BUILDING PERMIT APPLICATION, OWNER OR REPRESENTATIVE WILL

TEMPORARY STABILIZATION
1. ESTIMATED TIME OF CONSTRUCTION FOR THIS PROJECT IS FIVE MONTHS.
2. TEMPORARY CONTROL MEASURES AND ANY REQUIRED MA DEP SIGNAGE TO BE INSTALLED PRIOR TO ANY SITE WORK OR DEMO OF
EXISTING IMPROVEMENTS.
3. INSTALL PERIMETER FENCING, SILT CONTROLS, CONSTRUCTION ENTRANCES AND ESTABLISH STOCKPILE AREAS PRIOR TO SITE
WORK EXCAVATION. INSTALL SILT SAC PROTECTION AT CATCH BASIN ALONG HIGHLAND AVE INCLUDING CB APPROX 100' NORTH OF
SITE NEAR FOREST RIVER.
4. THROUGHOUT THE COURSE OF THE WORK, MAINTAIN EROSION AND SEDIMENTATION CONTROL STRUCTURES AS NECESSARY TO
KEEP ALL OPERATING AS INTENDED.
5. DRAINAGE SHALL BE PREVENTED FROM ENTERING SUBSURFACE INFILTRATION SYSTEM BEFORE THE SYSTEM IS COMPLETED AND
BACKFILLED.
6. TEMPORARY HAY BALES, STRAW WATTLES, OR SILT FENCES MAY BE REMOVED AFTER FINAL SOIL STABILIZATION AND BASE
PAVEMENT HAS BEEN INSTALLED AND ISSUANCE OF ANY REQUIRED APPROVAL FROM MUNICIPAL AUTHORITY.
7. CATCH BASIN INLET PROTECTION MUST REMAIN UNTIL BASE PAVEMENT IS INSTALLED AND ALL STOCKPILES ARE REMOVED AND NO
LONGER NECESSARY.
8. AT THE CONCLUSION OF CONSTRUCTION AND AFTER ALL FINAL STABILIZATION IS ACHIEVED, CLEAN ALL DRAINAGE STRUCTURES OF
ALL DIRT, DEBRIS, OR ANY LOOSE MATERIALS.
CONSTRUCTION SEQUENCE
1. CARE SHALL BE TAKEN TO PREVENT ANY UNSUITABLE MATERIAL FROM ENTERING ROADWAYS OR ANY DRAINAGE STRUCTURE.
2. PROTECT ALL INLETS, STOCKPILES AND UN-ESTABLISHED SOIL AREAS DURING STORMS AND PERIODS OF RAINFALL.
3. INSPECT ALL SEDIMENTATION CONTROL DEVICES PROMPTLY AFTER EACH RAINFALL AND REPAIR OR CLEAN OUT SEDIMENTATION AS
NECESSARY.
4. INSTALL ADDITIONAL CONTROL MEASURES AS NECESSARY AS CONDITIONS WARRANT.
5. AT THE END OF EACH WORK DAY, COVER ALL STOCKPILES AND INSURE ALL SEDIMENTATION CONTROL DEVICES ARE IN PLACE.
STOCKPILE SIDE SLOPES SHALL NOT BE GREATER THAN 2:1.
6. SEDIMENT SHALL BE REMOVED FROM SILT WATTLE BARRIER ONCE THE VOLUME REACHES

7. CONSTRUCT AND MAINTAIN A TEMPORARY CONSTRUCTION ENTRANCE PRIOR TO EXISTING PAVED AREAS ARE DISTURBED AND
CONSTRUCTION VEHICLES START TRACKING SEDIMENTATION INTO THE ROADWAY. CONSTRUCTION ENTRANCE(S) WILL BE
MONITORED AND CLEANED OF SEDIMENT AS NEEDED ON A DAILY BASIS. SWEEP THE CONSTRUCTION SITE AND ENTRANCES AS
NECESSARY TO PREVENT TRACKING OF SEDIMENTATION INTO THE ROADWAY.
8. TOPSOIL SHALL BE STRIPPED FROM AREAS TO BE DISTURBED, STOCKPILED IN APPROVED AREAS AND STABILIZED WITH TEMP
VEGETATIVE COVER IF TO BE LEFT STOCKPILED FOR MORE THAN 30 CALENDAR DAYS.
9. MAINTAIN PROPER DUST CONTROL THROUGHOUT THE PROJECT.
10. INSTALL A CONTINUOUS LINE OF SEDIMENTATION CONTROL ALONG THE PERIMETER OF ALL CONSTRUCTION ACTIVITY PER THE
EROSION CONTROL SITE PLAN.
11. PROTECT THE INLET OF ALL CATCH BASINS SUBJECT TO POSSIBLE RUNOFF FROM THE SITE WITH SEDIMENT FILTER AND INSPECT THE
FILTER AFTER ALL RAINFALLS AND ANY RUNOFF THAT REACHES THE CATCH BASIN.
12. INSURE PROPER HANDLING OF CONCRETE WASH OR OTHER CONSTRUCTION ACTIVITY TO PREVENT ANY SUCH WASH FROM REACHING
THE CATCH BASIN OR ROADWAY.
13. REMOVE SEDIMENTATION CONTROL AT ALL CBs UPON COMPLETION OF ALL WORK SO IT DOESN'T HINDER NORMAL DRAINAGE.
SEDIMENTATION CONTROL AND CONSTRUCTION OPN AND MAINT PLAN
1. UPON COMPLETION OF CONSTRUCTION, STORM WATER CONTROLS ARE TO BE MAINTAINED IN ACCORDANCE WITH FOLLOWING

TEST ALL FITTINGS. TEST SECONDARY PIPING AT 6PSI OR SMALLEST MAX RATING OF ANY COMPONENT (DO NOT EXCEED RATING
OF ANY COMPONENT) FOR MINIMUM OF 4 HRS AND SOAP TEST ALL FITTINGS. IF TESTING PRIMARY AND SECONDARY
SIMULTANEOUSLY, MAINTAIN MIN 10P PSI DIFFERENCE IN TEST PRESSURES.
9. TEST ALL VENT PIPING PRIOR TO BACKFILL IN ACCORDANCE WITH PIPING MFGR'S INSTALLATION INSTRUCTIONS. SEPARATE
PIPING FROM TANK BY INSERTING PLUG INTO EXTRACTOR VALVE AND OPEN OTHER TANK FITTINGS TO PREVENT BUILDUP OF
EXCESSIVE PRESSURE INSIDE THE TANK. VISUALLY INSPECT ALL BONDED JTS AND MAKE ANY NECESSARY REPAIRS PRIOR TO
PRESSURIZING PIPING SYSTEM. CHECK INTEGRITY OF JTS BY PRESSURIZING TO MAX 50 PSIG AND SOAP TEST ALL JTS.
10. ONCE THE SECONDARY LINE IS TESTED, TEST FITTINGS WILL BE LOOSENED AND MOVED BACK TO ALLOW COMMUNICATION
BETWEEN THE INTERSTITIAL SPACES AND THE SUMPS.
1. ALL SUMPS TO BE WATERTIGHT TO PREVENT LEAKS FROM OR WATER INTRUSION INTO SUMP. MEASURES PROPOSED TO
PREVENT WATER INTRUSION INCLUDE CROWNING OF CONC TANK PAD AROUND SUMPS, MANHOLES AND SUMPS WITH
WATERTIGHT GASKETED LIDS, AND WATERTIGHT BULKHEAD ENTRY FITTINGS

13. THE USE OF AUTOMATIC CREDIT READING DEVICES ARE PERMITTED PROVIDED THAT
THEY ARE INDICATED ON THE PLANS & COMPLY WITH THE REQMNTS OF 527 CMR 1.00

14. 225° (MAX) ELECTRIC THERMOSTAT DETECTORS SHALL BE INSTALLED ON 15 FOOT (MAX)
CENTERS IN A SINGLE LINE OVER THE CENTER OF THE ISLAND FOR ISLANDS UP TO 6 FT
WIDE. FOR WIDER ISLANDS 2 ROWS OF DETECTORS SHALL BE LOCATED 1 FT IN FROM EA
CURB. IF A CANOPY IS PROVIDED DETECTORS SHALL BE LOCATED ON THE UNDERSIDE
OF THE CANOPY. IF THERE IS NO CANOPY, THE DETECTOR(S) SHALL BE MOUNTED TO
THE UNDERSIDE OF AN 18"x18"x2" HEAT COLLECTOR, WHICH SHALL BE LOCATED NO
HIGHER THAN THE NOZZLES.
14. FIRE EXTINGUISHERS SHALL BE A MINIMUM 40BC HIGH FLOW.
15. RAISING OR LOWERING OF AN EXSTG SUPPRESSION SYS OR TEMP RELOCATION OF
CONTROL CONSOLE MAY BE COMPLETED ONLY AFTER WRITTEN NOTIFICATION TO AND
WRITTEN ACKNOWLEDGEMENT FROM THE HEAD OF THE LOCAL FD AND STATE FIRE
MARSHAL OFFICE.
16. CHANGING HEAT DETECTORS TO THOSE SPECIFIED IN CURRENT GUIDELINES, OR
REPLACING DISPENSING NOZZLES WITH "HOLD OPEN" NOZZLES SHALL NOT REQUIRE
NOTICE OR APPROVAL FROM STATE FIRE MARSHAL OFFICE.
17. A COMPLETE RESUBMITTAL IN ACCORDANCE WITH THEN CURRENT SELF-SERVICE
GUIDELINES IS REQUIRED IN THE FOLLOWING CASES:
17.1. REPLACEMENT OF THE FIRE SUPPRESSION SYSTEM
17.2. CHANGE IN METHOD OF DISPENSING (I.E.- CHANGING FROM SPD TO MPD
DISPENSERS)
17.3. REPLACEMENT OR RECONSTRUCTION OF THE DISPENSING ISLAND
17.4. MATERIAL CHANGE IN ENVIRONMENTAL CONDITIONS OR HAZARD AREA (I.E.-
CHANGE THAT IMPACTS ATTENDANT ABILITY TO OBSERVE THE FUELING OPERATION)
18. THE CONTRACTOR SHALL VISIT THE SITE AND BE RESPONSIBLE FOR FIELD
CONFIRMATION OF ALL ITEMS ON THE PLAN AND SHALL NOTIFY THE DESIGNER IN
WRITING OF ANY DISCREPANCY BEFORE PROCEEDING. THE CONTRACTOR SHALL BE
RESPONSIBLE FOR THE INSTALLATION OF THE ENTIRE SYSTEM AND SHALL COMPLY
WITH THE REGULATIONS SET FORTH BY THE PREVAILING MASSACHUSETTS CODE AND

"GUIDELINES FOR SELF-SERVICE MOTOR FUEL FACILITY GUIDELINES", 527 CMR 1.00 FIRE
REGS, NFPA 17, & THE SYS MFGR UL LISTING, SYS MANUALS, & LOCAL AND STATE
APPROVALS FOR THIS SYSTEM.
2. ALL MA SELF SERVICE DISPENSING AREAS OPEN TO THE PUBLIC SHALL BE EQUIPPED
WITH A LISTED UL-1254 COMPLIANT PRE-ENGINEERED DRY CHEMICAL EXTINGUISHING
SYSTEM, INCLUDING END OF ISLAND PROTECTION; AND DESIGNED & INSTALLED TO
PROTECT THE ENTIRE SPILL & HAZARD AREA.
3. STATE REQUIRED PLAN STATEMENTS:
3.1. SELF-SERVE EXTINGUISHING SYSTEMS SHALL BE INSTALLED IN ACCORDANCE WITH
STATE FIRE MARSHAL APPROVED PLANS.
3.2. ALL EXTINGUISHING SYSTEMS, DISPENSING SYSTEMS, AND CONTROL EQUIPMENT
SHALL BE UNDERWRITERS LABORATORY (UL) LISTED.
3.3. CREDIT CARD READING DEVICES ARE PROPOSED AT THIS LOCATION AND ARE
SUBJECT TO 527 CMR 1.00 SECTION 42.7.4.5 (1)(K):
3.3.1. EA SALE SHALL BE INDIVIDUALLY AUTHORIZED BY THE SELF-SERVICE
ATTENDANT
3.3.2. THE AUTOMATIC CREDIT CARD READING DEVICE SHALL NOT BE USED AS
PHYSICAL AUTHORIZATION FOR THE DISPENSING OF MOTOR FUEL
3.3.3. THE AUTOMATIC CREDIT CARD READING DEVISES ARE INCLUDED ON PLANS
SUBMITTED TO AND APPROVED BY THE STATE FIRE MARSHAL (SEE NOTES AND
LEGEND)
3.4. FAILURE TO CONSTRUCT, INSTALL, OR MAINTAIN ANY EQUIPMENT, DEVICES,
SYSTEM, OR PROCEDURE INDICATED ON THE APPROVED PLANS SHALL BE CAUSE
FOR THE REVOCATION OF THIS APPROVAL
3.5. PLANS APPROVED BY THE MARSHAL ARE ON THE CONDITION THAT THEY COMPLY IN
ALL RESPECTS TO 527 CMR 1.00. ANY OMISSIONS OR ERRORS ON THE PLANS DO
NOT RELIEVE THE APPLICANT OR INSTALLER FROM COMPLYING WITH ALL
APPLICABLE REQUIREMENTS OF 527 CMR 1.00.
3.6. STATE FIRE MARSHAL APPROVAL OF PLANS TO CONSTRUCT/RENOVATE A
SELF-SERVICE FACILITY BECOMES NULL AND VOID ONE YEAR FROM THE DATE OF
THE APPROVAL.
4. ALL FIRE SUPPRESSION SYSTEMS SHALL BE INSTALLED & MAINTAINED BY COMPANIES &
PERSONS HOLDING REQ'D CERTIFICATES OF COMPETENCY & REGISTRATION ISSUED BY
THE MARSHAL.
5. FIRE SUPPRESSION SYS SHALL BE INSTALLED & MAINTAINED IN ACCORDANCE WITH THE
PROVISIONS OF NFPA 17 (SEE 527 CMR 1.00 CH 2 FOR CURRENT EDITION), AND THE
MFGR'S INSTALLATION & MAINTENANCE MANUAL.
6. CLOSED CIRCUIT TV (CCTV) COVERAGE IS REQUIRED ON ALL FUELING POSITIONS. CCTV
MONITORS SHALL DISPLAY ALL CAMERA ANGLES SIMULTANEOUSLY ON THE SCREEN
WITH A MIN FRAME OF 5" BY 6.5" PER CAMERA. CCTV SHALL DISPLAY IMAGES IN COLOR.
7. AN INTERCOM SYS IS REQ'D BETWEEN THE CONTROL CONSOLE AND DISPENSING
ISLAND.
8. THE CONTROL CONSOLE WILL BE LOCATED IN DIRECT LINE OF SIGHT AND WITHIN 200
FEET OF THE MOST REMOTE DISPENSER. DISTANCES GREATER THAN 200 FT MAY BE
APPROVED AT SOLE DISCRETION OF THE STATE FIRE MARSHAL.

NARRATIVE
THE PURPOSE OF THIS WORK IS TO INSTALL A DRY CHEMICAL FIRE SUPPRESSION SYS
MEETING UL-1254 AT THIS NEW TO INDUSTRY SELF SERVICE MOTOR FUEL FACILITY.
THE PROPOSED FACILITY INCLUDES 6 FUEL DISPENSER (12 FUELING POSITIONS) INCLUDING
DIESEL AND DEF, ALL DISPENSERS INCLUDE CREDIT CARD READERS. THE ATTENDANT IS
LOCATED IN A CONVENIENCE STORE AND LESS THAN 115 FEET FROM THE FURTHEST
DISPENSER.
PLBs (POSITIVE LIMITING BARRIER GROOVES) WILL BE INSTALLED AROUND THE ENTIRE
FUELING AREA. CAMERAS (CCTV) AND SIGNAGE WILL ALSO BE INSTALLED AT EA DISPENSER
TO MEET STATE FIRE MARSHAL GUIDELINE REQUIREMENTS. ALL EQUIPMENT WILL BE
INSTALLED PER THE MFGR'S PRE-ENGINEERED DESIGN MANUAL.
THE SYSTEM WILL PROVIDE BOTH AUTOMATIC AND MANUAL ACTIVATION SWITCHES. THERE
WILL BE HIGH TEMP SENSORS INSTALLED IN THE FUEL DISPENSER AREAS. SYSTEM
OPERATION, MAINTENANCE, AND TECHNICAL SPECIFICATIONS ARE PER THE MFGR DESIGN
MANUAL.
GRADES IN FUELING AREAS TO BE AS LEVEL AS POSSIBLE,
AVERAGE GRADES FOR THIS SITE APPROX 2.5% - 4%

1. THE GENERAL PURPOSE OF THIS WORK IS TO CONSTRUCT A NEW GAS
STATION AND CONVENIENCE STORE.
2. STORMWATER IMPACTS OF THE PROPOSED DEVELOPMENT HAVE BEEN
