Dear Owner:
On Monday, June 30, 2025, David Greenbaum, Health Agent, for the Salem
Board of Health, Officer Musto, and Danielle Csogi visited your property to
conduct a well-being check. We spoke with you, and you granted permission for
us to enter your unit. While in the unit conditions deemed to endanger or
materially impair the health or safety of the occupant were observed.
Specifically:
• The bathroom is soiled with human feces on the toilet and the floor
• The bed appears to covered in urine and/or feces.
• There is trash and debris littering the unit.
• There is rotting food litter the unit
• There is a mouse infestation in the unit, as evidenced by the excessive
amounts of mouse feces all over the floors
• There is a fly and fruit fly infestation.
• There are no working smoke detectors or carbon monoxide detectors in
the unit.
• There is a back door that has no porch creating a fall hazard from the third
floor.
Based upon these findings and in accordance with 105 CMR 410.630(B), the
Board of Health determines that the danger to the life or health of the
occupants is so immediate, condemnation is ordered immediately and the
hearing requirements of 105CMR 410.800 are waived. All occupants of the
residence are ordered to vacate the unit immediately. If any person refuses
to leave the dwelling they may be forcibly removed by the Board of Health
or by local police authorities on the request of the Board of Health.
If after one year from the issuance of an order to secure compliance with the
minimum standards set forth in 105 CMR410.000 has not been affected, then the
Board of Health may cause the dwelling to be demolished. Any costs borne by
the Board of Health are a debt to the City and shall be recoverable from the
owner in an action of contract.
No dwelling or portion thereof, which has been condemned and placarded as
unfit for human habitation, shall again be used for human habitation until written
approval is secured from, and the Board of Health removes such placard. No
person shall deface or remove the placard, except the Board of Health shall
remove it whenever the defect or defects upon which the condemnation and
placarding action was based have been eliminated.
Should you be aggrieved by this Order, you have the right to request a hearing
before the Board of Health. A request for such a hearing must be received in
writing in this office of the Board of Health within seven (7) days of receipt of this
Order. At said hearing, you will be given the opportunity to be heard and to
present witness and documentary evidence as to why this Order should be
modified or withdrawn. You may be represented by an attorney. Please also be
informed that you have the right to inspect and obtain copies of all relevant
inspection or investigation reports, orders, and other documentary information in
the possession of this Board, and that any adverse party has the right to be
present at the hearing.

Message Good evening,
Today i responded to 18 Roslyn St unit 3 for a medical call. We
gained entry to the apartment and found unsanitary conditions
inside of the apartment. The unit is occupied by a 62 year old
man who lives alone. The apartment has trash all over the
ground, discarded cigarettes, rotten food and fruit flies all over
the apartment. Elder services has been contacted by us and
SPD.Attatched are photos of the conditions

Police Department Community Impact Unit (CIU) permission to enter my condo
located at 18 Roslyn Street #3 for the purposes of inspecting the cleanup and
checking the compliance with the condemnation order issued on June 30, 2025.
This inspection can be done in my absence.

Dear Sir/Madam:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum Standards
of Fitness for Human Habitation, a recent reinspection was conducted of your property located
at 18 Roslyn Street#3, conducted by David Greenbaum, Health Agent of the Salem Board of
Health on July 14, 2025.
Many of the critical violations that could.endanger or impair the health or safety of the occupants
have been corrected. The following conditions must still be met,
• Smoke and carbon monoxide detectors must be installed in accordance with all
applicable codes inside the unit
• The back door to the roof must be secured so that the roof is not accessible.
• The owner agrees to continue homecare services through Age Span to maintain the unit
in a clean and sanitary manner.
The Board of Health deems the unit habitable at this time. Therefore, the order of
condemnation issued by the Board of Health is hereby lifted.
