Chapter 111, sections 122-127B Dear.Sir or Madam: An inspection of your property at 11 Sylvan Street in Salem was conducted by David Greenbaum, Health Agent for the Board of Health on January 29, 2025, pursuant to General Laws Chapter 111, Sections 122-127B. The following observations were made: There is an accumulation of yard waste, old furniture, loose trash, and miscellaneous trash and debris littering this property. Additionally, this property continues to be completely overgrown. Conditions are ripe for rodent activity and harborage and complaints of their presence on this property have been received by this office. Therefore, the Salem Board of Health deems a public health nuisance exists on your property and must be abated. You are hereby ordered to: 1. Clear overgrown vegetation. All overgrown vegetation must be trimmed or removed to eliminate hiding places and breeding grounds for pests and rodents. 2. Take Rodent Control Measures: Professional pest control services must be engaged to address and eradicate the rodent infestation and removal of any rodent attractants. 3. Remove all Trash and Debris: All trash, debris, and waste materials must be collected and properly disposed of. The property should be kept clean and free of litter to prevent future health hazards. YOU MUST ACT NOW. You must complete all corrective actions within.ten days of receipt of this order. Failure to comply within ten days may result in the City taking corrective action by hiring contractors to correct the violations and impose a lien on your property to recover the costs of such action. It may also result in other,legal action being taken against you, including criminal prosecution. NOTICE: You have the right to request a hearing before the Salem Board of Health to contest this Order. Your request must be in writing and received by.the Board within five days of receiving this order. Deliver your request to David Greenbaum, Health Agent, Salem Board of Health, 98 Washington Street, 3rd Fl., Salem City Hall Annex, Salem, MA 01970. At said hearing, you will be given the opportunity to be heard and to present witness and documentary evidence as to why this Order should be modified or withdrawn. You may be represented by an attorney. You have the right to inspect and obtain copies of all relevant inspection or investigation reports, orders, and other documentary information in the possession of the Board of Health and that any adverse party has the right to be present at the hearing.
