In accordance with M.G.L. c. 111, §§ 3 and 127 A, as well as Massachusetts Department of Public
Health Regulations 105 CMR 410.000: Minimum Standards of Fitness for Human Habitation (State
Sanitary Code, Chapter II), an inspection was made of your property at the above address on
11/26/2025.
The attached Inspection Report lists the violations noted during the inspection and include the
timeframes in which the violations must be corrected.
You are hereby ORDERED to correct these violations within the noted time limit. Failure to comply
within the allotted time period may result in a criminal complaint against you.
Violations identified as CDE are Conditions Deemed to Endanger or Impair the Health or Safety
of the occupants, which may permit the occupant of the dwelling to exercise one or more of
their statutory remedies.
You have a right to request a hearing before the Board of Health. This request must be made by you, in
writing, and filed within seven days after the day this Order was served.
If you request a hearing, all affected parties will be informed of the date, time and place of the hearing
and of their right to inspect and copy all records concerning the matter to be heard. The petitioner has
the right to be represented at the hearing.
Every occupant of a dwelling unit or rooming unit shall give the owner, their agent, or employees, upon
reasonable notice, reasonable access, if possible, by appointment, to the dwelling unit or rooming unit
for the purpose of making repairs necessary to satisfy this Order.
A re-inspection will be conducted to confirm compliance with this Order. Compliance means meeting all
the requirements of 105 CMR 410.000. It shall also mean correcting any violations of 105 CMR 410.000
in a workperson like fashion and restoring all parts of the dwelling, or unit thereof, to the condition they
were in before occurrence of any such violations. Compliance shall also mean in those cases where
licenses or permits are required to perform work necessary to correct the violations, such as, but not
limited to building, plumbing and wiring that the appropriate official certifies that the work has been
completed in accordance with applicable laws and regulations.

Any person who shall fail to comply with any order issued pursuant to the provisions of the State
Sanitary Code, Chapter II, 105 CMR 410.000 shall upon conviction be fined not less than ten ($10)

Observations: In the doorway leading into the bedroom,the strike plate is missing and behind the strike plate hole there is
a recess that has been filled with plastic material. Repair the strike plate hole and provide a strike plate for this doorway.

accepted standards so they are in good repair and in every way fit for the intended use, including: (2) Free from holes,
cracks, loose plaster, or defects that renders the area difficult to keep clean, create an injury risk, or provide an entry or

Observations: The occupant informed the inspector that in the past he observed pests in the bedroom's door frame
coming out of the striker plate hole.At the time of inspection the inspector did not observe pests in the bedroom door
frame's strike plate hole or anywhere within the dwelling unit. The inspector advises the property owner and property
manager to monitor the property for pests.

Observations: The occupant made a complaint about the heating system being inefficient. At the time of inspection the
inspector used an air probe and observed the following:
- Heating system turned on at 9:00am
- Living Room temperature: 68.2*F at 9:24am
- Bathroom temperature:68.2*F at 9:27am
- Kitchen temperature:68.2*F at 9:32am
-Bedroom temperature: 68.4*F at 9:34am
During the Heating Season heating systems have to be able to provide heat at temperatures between 68.0*F and 78.0*F
between 7:00am and 11:00pm, and 64.0*F and 78.0*F between 11:01 pm and 6:59am.

Observations: The occupant made a complaint about air passing around the glass in the living room windows and the
bedroom sliding doors.At the time of inspection the occupant turned on hot water in the bathtub to show where
condensation lay on the living room windows. On the same day condensation was observed on bedroom sliding doors on
the right.
On November 18, 2025, the inspector returned with Stephen Casey, Jr., the Regional Public Health Trainer,to inspect the
windows and sliding doors further. It was determined the glass panes in the windows and sliding doors were intact at the
time. It was also determined that while there was some air flow around the living room window in the top middle and top left
corner areas, exterior wind did not appear to enter through these areas. Also, it was noted that there was caulking and felt
weather stripping added around the edges of the sliding doors and the sliding doors'frame by the occupant. No
condensation was observed on the sliding doors' glass at this time.
The inspector advises the property owner and property manager to monitor the windows and sliding doors for deterioration.

Observations: In the hallway adjacent to the kitchen, the entry door of the dwelling unit has a gap between the bottom of
the door and the floor large enough to allow the entry of pests. Either adjust the door or provide weather stripping for this
door so that the gap between the bottom of the door and the floor is no longer large enough to allow the entry of pests.
