An inspection of your facility on Wednesday November 1, 2006 revealed the violations
listed below.
ORDER TO COMPLY: Since these conditions are contrary to law, you must correct
them upon receipt of this notice. An inspection to determine compliance with

Smoke detectors in common hallways had no power, the breaker at the electrical
panel was thrown.
Smoke detector breaker at electrical panel is to be secured with a tamper proof
lock and clearly marked in red for identification.
Smoke detectors appear to be older than 10 years, all smoke detectors older
than 10 years are to be replaced according to manufacturer' s specifications .
Smoke detectors are to be located in the immediate vicinity of all sleeping
areas, any smoke detectors that are missing or have been removed-are to be
replaced/installed as required.
11.303. Storage, Use & Handling of Misc. Combust 0 0
Large accumulation of combustible storage in common hallways.
No storage in common hallways is permitted at any time, hallways are to be kept
free and clear at all times .
Storage of discarded water heater and out of use heating equipment in basement.
All items to be removed forthwith.

rapid entry system, with keys to all exterior doors and all common areas,
installed on exterior of building easily visible from the street. Knox Box to
be ordered through the Salem Fire Prevention Bureau.
