Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, please contact the Salem Board of Health at(978) 741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order, you have the right to request a hearing before the Board of Health. A request for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order. At said hearing, you will be given an opportunity to be heard and to present witness and documentary evidence as to why this Order should be modified or withdrawn. Violations Based upon a tenant complaint,an inspection was conducted on September 11,2023 in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon re-inspection,the following were noted: Violation numbers 1 through 4 on the September report have been corrected. 14 days .500 1. Wall to the left of the bathroom cabinet is patched but has bumpy and bubbling areas of plaster and paint. Scrape and properly plaster and repaint this wall so it is smooth and in good condition. Owner to correct this violation within 14 days. 7 days .500 2. Bathroom window does not stay up when opened. Repair window so it is able to open, stay open,and close properly. Owner to correct this violation within 7 days. 7 days .500 3. Left window in bedroom has a loose bottom sash. Repair window so sash is secure and window can be properly opened and closed. Owner to correct this violation within 7 days. 7 days .500 4. Right window in bedroom appears to be screwed shut. Unscrew and repair this window so it can be properly opened and closed. Owner to correct this violation within 7 days. 7 days .351 5. Showerhead is leaking and the tub spout is pulling away from the wall. Make necessary repairs so showerhead does not leak anymore and repair tub spout so it is properly installed in the wall and there are no gaps around it. Owner to correct this violation within 7 days. 7 days .500 6. Left living room window does not stay up when opened. Repair window so it is able to open, stay open, and close properly. Owner to correct this violation within 7 days.
