Dear Ms. Saris,
On Wednesday, January 22, 2025, 1 conducted a walkthrough of LEAP for Education
located at 35 Congress Street, Suite 102 to assess the facilities capacity to work with
a third-party vendor to provide a hot, pre-cooked meal five days a week.
Based upon my observations at that time the Salem Board of Health finds that your
facility meets the requirements of the food code to offer this type of meal through an
outside vendor.
Please provide the name of the vendor you will be using to provide the meal to the
Board of Health.
This approval will be valid until December 31, 2026, as long as there are no changes
to the existing conditions.of your facility.
