Dear Ms. McDermott:
At the request of the Salem Fire Department and in accordance with Chapter 111,
Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000; State
Sanitary Code, Chapter I: General Administrative Procedures and 105 CMR 410.000:
State Sanitary Code, Chapter 11: Minimum Standards of Fitness for Human Habitation,
an inspection was conducted of your apartment at 13 Palmer Street by David
Greenbaum, Health Agent on January 9, 2020 at 1:30 P.M. You reside in this
apartment with four other adults some of whom are your children. At the time of
inspection, you were found to be in violation of the following sections of 105 CMR
410.000 Chapter II Minimum Standards of Fitness for Human Habitation of the State
Sanitary Code:
410.352(B):
The stove is unclean and not kept in a clean and sanitary manner. Occupant
must clean and maintain the stove in a clean and sanitary manner.
The refrigerator/freezer has an accumulation of food debris, spills and splatter.
Occupant must clean the refrigeratodfreezer and maintain in clean and sanitary
manner.

The apartment is extremely unsanitary, there is trash piled and stored in the
kitchen. There are food spills and splatter all over the kitchen; the sink is full of
dirty dishes with an infestation of drain flies in the kitchen; the cat box is
overflowing with cat excrement. There are rooms piled with trash and
miscellaneous debris throughout this unit. Occupant must clean and maintain
this unit in a clean and sanitary manner.
A reinspection of our unit will be conducted on Thursday, January 30, 2020 at
10:OOAM. The occupants must have the above violations corrected by that time.
Failure on your part to comply within the time specified timeframe will result in a
complaint being sought against you in the Northeast Housing Court. Time for
compliance begins with receipt of this Order.
Should you be aggrieved by this Order, you have the right to request a hearing before
the Board of Health. A request for said hearing must be received in writing in the office
of the Board of Health within 7 days of receipt of this Order. At said hearing, you will be
given an opportunity to be heard and to present witness and documentary evidence as
to why this Order should be modified or withdrawn. You may be represented by an
attorney. Please also be informed that you have the right to inspect and obtain copies of
all relevant inspection or investigation reports, orders and other
documentary information in the possession of this Board, and that any adverse party
has the right to be present at the hearing.
