Joe, We have undertaken an assessment of the proposed onsite wastewater holding tank design provided for the above address dated 3-4-24 and offer the following comments: 1. Provide a DSCP application with the submission 2. Provide soil evaluation forms with the submission 3. Confirm the size of the existing septic tank which is proposed to be re-used based on field measurements 4. Depict the location of the proposed alarm 5. Provide a detail depicting the internal tank components to guide the installer during construction 6. Clarify why there is a vent detail provided on the drawing but not specified elsewhere 7. Notes 9& 10 imply the building sewer is to be replaced yet this is not depicted elsewhere. Clarify what is intended for the building sewer 8. City tax records depict the property boundary differently than shown. They indicate the southern portion of what is depicted as the parcel is not actually 8 Wyman Avenue but rather 1 Wyman Street, and that 1 Wyman Street is owned by the City of Salem. If the tax records are correct, depict the property boundaries. Additionally, if they are correct,then the proposed onsite wastewater holding tank might be located on municipally owned land and if so provide an agreement with the City to maintain the tank on the land and also request needed variances or Local Upgrade Approvals as indicated in Title 5. If the tax records are not correct, please explain. Also attached is a map of the neighborhood provided by the designer. Testing occurred on lots identified as 14& 11 on this map. The findings were that much of both of these parcels had shallow depth to ledge and those areas are not considered suitable for placement of a soil absorption system under the Massachusetts onsite wastewater rules known as Title 5. Small areas on each lot were found to have suitable depth of acceptable material. My notes should be retained by your office so that they can be compared to the submission provided by the applicant to confirm accuracy of their information.
