At your request the Board of Health invites you to the next Board of Health meeting to be heard
relative to your request for a hearing on the order issued to you July 25, 2023, regarding the property
located at 5 Laurent Road, Salem.
The meeting is scheduled for Tuesday, November 14, 2023 @ 7:00 p.m. The meeting will be held at
City Hall Annex, 98 Washington Street, in the first-floor large conference room.
At said meeting, you will be given an opportunity to be heard relative to your issues and concerns
regarding the order issued for your property located at 5 Laurent Road. You have the right to be
represented by legal counsel. All opposing parties also have the right to be present and to be
represented by legal counsel.

On Tuesday, October 10, 2023, David Greenbaum, Health Agent, for the Salem
Board of Health served an administrative search warrant on your property
located at 5 Laurent Road. Also present at this time were representatives of the
Salem Police and Fire Departments and the Salem Building Commissioner
Inspection of the dwelling was conducted and the following conditions that could
materially impair the health and safety of the occupants were noted:
• There are exposed electrical wires in the house.
• The oil tank is rusted and leaking.
• There is a severe mouse infestation throughout the house.
• There is a severe mold infestation throughout the house.
• All the windows and the front,door are completely obstructed and blocked
from egress.
• The refrigerator is not working.
• The stove does not appear to be working.
• There is trash and debris littering the house.
• The toilet does not work properly.
Based upon these conditions, the Building Commissioner has revoked the
occupancy of this dwelling and the Salem Board of Health deems this
dwelling unsafe and unfit for human habitation. Based upon these findings
and in accordance with 105 CMR 410.630(B), the Board of Health
determines that the danger to the life or health of the occupants is so
immediate, condemnation is ordered immediately and the hearing
requirements of 105CMR 410.800 are waived. All occupants of the
residence are ordered to vacate the unit immediately. If any person refuses
to leave the dwelling they may be forcibly removed by the Board of Health
or by local police authorities on the request of the Board of Health.
If after one year from the issuance of an order to secure compliance with the
minimum standards set forth in 105 CMR410.000 has not been affected, then the
Board of Health may cause the dwelling to be demolished. Any costs borne by
the Board of Health are a debt to the City and shall be recoverable from the
owner in an action of contract.
No dwelling or portion thereof, which has been condemned and placarded as
unfit for human habitation, shall again be used for human habitation until written
approval is secured from, and the Board of Health removes such placard. No
person shall deface or remove the placard, except the Board of Health shall
remove it whenever the defect or defects upon which the condemnation and
placarding action was based have been eliminated.
Should you be aggrieved by this Order, you have the right to request a hearing
before the Board of Health. A request for such a hearing must be received in
writing in this office of the Board of Health within seven (7) days of receipt of this
Order. At said hearing, you will be given the opportunity to be heard and to
present witness and documentary evidence as to why this Order should be

Deputy Schaeublin (C-6)called me to respond with him to assist the police on a wellbeing check of a residence to
access the current fire safety conditions. Upon arrive C-6,members of the police department and I entered the
residence after being invited in by one of the residences. We entered the kitchen area;the area was hard to navigate
through due to the number of various items on the floor. I noticed a foul odor in the kitchen, most likely from trash and
other waste in the kitchen. I walk to the main hallway of the residence there were numerous electrical type wires hung
at eye level on the wall.These wires pose a fire risk due the unknow condition of the wire.There was one"smoke
alarm"reported by one of the occupants on the wall was not working any longer. I didn't discern the item as a smoke
alarm and am unsure of what the item was. The house didn't have any visible CO alarm as required by code within 10
feet of the bedrooms. I was not allowed to look in the bedroom to determine if the bedrooms had working smoke
alarms. I observed one of the police officers checking the water in the bathroom. I heard him state the hot water wasn't
working but the cold water was.The toilet didn't have running water and I observed human waste remaining in the
bowl. The bathroom had the strong odor of urine. I walk back to the kitchen area and looked into the front room.
wasn't able to enter the room since the room was full of various stored items. Passage through the front door of the
house was block by the large number of items stored in the front room. It appeared that most of the items were
combustible in nature and in the event of a fire would contribute to a heavy fire load in the building.As I walked back
though the kitchen, I observed on most flat surfaces(floor, counter and shelves)a large quantity of black pellets that
appeared to be mouse droppings.An elderly lady lives in the residence and the current conditions of the interior of the
building would make any type of assistance by first responders difficult to complete. Self-rescue in the event of a fire
would be impossible.All residence of the building are in danger due to the lack of smoke and CO alarms present as
well as the lack of proper egress pathway in the event of a fire.

Car 3(Spare Deputy Car)
SPD requested the fire department to the scene.They were there to assist Agespan nurses that came for a home visit
to do a check on a resident. Inspector Geary and I entered through the kitchen on the right side of the home.There
were many items in the kitchen that made navigating the space difficult. Inspector Geary went down the hall and I went
toward the front of the house.There was excessive storage of boxes and made it impossible to enter the front room
which is where the front door is located. Front windows had become dislodged so weather was getting into the home.
Combustible material was all over the stove, I asked that it be removed and the resident told me he would do it today.
We observed no working smoke or carbon monoxide detectors in the home.

Risk To Investigator explanation:There could be a risk to a PS investigator. Terrance Talbot can be erratic. Sean Talbot admitted that at one
point he owned a firearm but said he turned it in to Salem PD. This is currently being investigated by Salem PD.Describe the elder's current
physical,emotional, and mental status including medical issues, medications the elder takes,services the elder receives,any confusion or
memory loss, and whether the elder has the ability to make his/her own decisions. Describe the type of housing the elder resides in (ex.
private home, apartment,assisted living, etc.); who lives with the elder(provide names and contact information if possible);any concerns
about the physical condition of the elder's housing (be as specific as possible).On 1/29/20 the Salem Board of Health received a report of
unsanitary conditions at 5 Laurent Road, Salem MA. This is a single family,owner occupied home. As the board of health has had previous
interactions with this property we visited the property on 1/29/20 with Salem PD. At that time there was no answer at the door. The Salem
Board of Health observed numerous trash bags in the yard and has issued an order to clean the property. On January 30,2020 the Salem
Board of Health and Salem PD visited the house again and made contact with Sean and Terrence Talbot. They allowed access to the house
and to speak to their mother,Anne Rita Talbot. Anne Rita is legally blind and suffers from diabetes. She is bed ridden and has had toes/feet
amputated due to the diabetes. She takes numerous medications,too many to count. The house is cluttered with trash and debris and there
is some evidence of hoarding. Anne Rita's bedroom has an accumulation of stuff including trash. She is completely dependent on her sons
for care and there is some evidence that they are unable to care for themselves let alone a bedridden, legally blind elder.Explain why you are
reporting today and if you suspect any of the following: physical abuse, emotional abuse, sexual abuse,caretaker neglect,financial
exploitation,and/or self-neglect. Provide as much detail as possible including, names,dates, locations,and any injury or harm to the elder. If
this is a financial exploitation case, please provide detailed information regarding the elder's finances.This report is being filed for many of the
reasons stated above. The Salem Board of Health is concerned that Anne Rita Talbot is unable to receive the care she needs from only her
two sons. Explain the elder's current level of risk. Is the abuse/neglect a long term or ongoing problem? Can the elder call for help if
necessary?The risk level is currently moderate to high.Does the alleged perpetrator still have access to the elder or the elder's finances
(financial exploitation)?The elders son Sean Talbot controls the finances. It appears that the family survives on Anne Rita's social security
and that there is no other source of income.Has the elder been admitted to the hospital, if so how long will the elder remain?Any concerns
around losing housing(eviction,foreclosure,etc.)If the elder is being evicted,when is the eviction date?No,the elder is still in the
home.Provide the name and contact information of the elder's Primary Care Physician (PCP).Dr. Marc Messenger

Dear Sir or Madam,
On January 29, 2020, David Greenbaum, Health Agent and Elizabeth Gagakis,
Senior Sanitarian for the Salem Board of Health along with officers from the
Salem Police Department Community Impact Unit visited your proper located at 5
Laurent Road in the City of Salem. During this visit several conditions were
observed that are very concerning.
Observed at that time were: piles of black trash bags all over the property. There
are empty cardboard boxes, wood, furniture, tools and toolboxes as well as a
snow blower and other miscellaneous trash and debris on the property.
Additionally, the front stairs are collapsing and there are several areas around
the roof that are open and allow the entrance of rodents and animals into the
building. The building inspector has been notified of these issues. (Pictures

On January 29, 2020, David Greenbaum, Health Agent and Elizabeth Gagakis,
Senior Sanitarian for the Salem Board of Health along with officers from the
Salem Police Department Community Impact Unit visited your proper located at 5
Laurent Road in the City of Salem. During this visit several conditions were
observed that are very concerning.
Observed at that time were: piles of black trash bags all over the property. There
are empty cardboard boxes, wood, furniture, tools and toolboxes as well as a
snow blower and other miscellaneous trash and debris on the property.
Additionally, the front stairs are collapsing and there are several areas around
the roof that are open and allow the entrance of rodents and animals into the
building. The building inspector has been notified of these issues. (Pictures
