320 LAFAYETTE STREET UNIT 206 REMAINING VIOLATIONS LETTER 9-23-2025 Ito.: CITY OF SALEM, MASSACHUSETTS S BOARD OF HEALTH 98 WASHINGTON STREET 3R"FLOOR ptlbbcHealth Prevent.Promote.Protect. TEL. (978) 741-1800 DOMINICK PANGALLO health@salem.com DAVID GREENBAUM,RS MAYOR HEALTH AGENT September 23, 2025 Park Towers, LLC 100 Washington Street, Office 1 Salem, MA 01970 Dear Mr. Almeida: In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum Standards of Fitness for Human Habitation, a recent re-inspection was conducted of your property located at 320 Lafayette Street Unit 206, conducted by Jeffrey Barosy, Sanitarian of the Salem Board of Health on September 22, 2025. At that time the following items were still outstanding: Living Room: - According to the occupant, water and air still penetrate the living room through the living room window. The inspector could not confirm this at the time of inspection. - According to the occupant, the ceiling in front of the living room window still leaks water. The inspector could not confirm this at the time of inspection. However, the occupant showed the inspector video of water leaking from the living room ceiling after the most recent repairs were made. The videos shown to the inspector lead the inspector to believe whatever work was performed was not sufficient to permanently stop the water leakage and air penetration in the living room ceiling, the living room windows, and the bedroom sliding doors. Porch: - According to the occupant, water and air still penetrate the bedroom through the sliding doors leading to the porch. The inspector could not confirm this at the time of inspection. Please correct the outstanding violations and contact this office to schedule a final re-inspection of this apartment. All other violations noted in the inspection report of June 26, 2025, have been corrected. Thank you for your cooperation in this matter.
