Dear Mr. Patel,
Prism Energy Services performed a lighting installation at the above location. Our auditor, Dan Doherty received a
phone call from you stating that the pole lights were shining onto the street and that the City of Salem had issued a
violation notice that needed to be complied with. We were provided with a copy of the letter from the City. a.
Jerry Giusti,our VP of Project Management spoke with Thomas St. Pierre,the Building Commissioner,who
explained the issue. Jerry assured the Inspector that Prism would return within the next few days to re-direct the
fixture heads onto the property and away from the street and adjacent buildings. Mr.St. Pierre was very helpful,
reasonable,and accepted our time frame for responding.
Upon sending the installing contractor back to the site,they immediately informed Prism that upon arriving,that
they found that the fixtures(on adjustable arms)had been re-directed after the installation to be pointed in a
different direction than the way they were installed. The contractor properly re-adjusted the fixture heads back to
the appropriate position, pointing onto the property,as they were originally installed.
Upon the work being completed,Jerry again contacted Mr.St. Pierre to confirm that the correction had been
made. The Inspector thanked Prism for prompt attention to correct the fixture head.
Subsequently, Prism learned that the fixture heads had been re-adjusted again from the proper direction. We
bring this to your attention to clarify that Prism has now properly adjusted the fixture heads twice:once during the
original installation and again after the call requesting re-adjustment due to the Building Commissioner's
complaint.
We are not certain why these fixtures are being re-directed or by whom. This letter shall serve to document that
Prism installed the fixtures correctly at the original installation,as well as returning to re-direct them at a
subsequent call back. As far as Prism is concerned,whatever the circumstances,our responsibility for the proper
installation has been fulfilled and confirmed. We consider the matter closed,as it pertains to our company.
Should you have future issues or complaints,it will not be appropriate for Prism to respond,as it will not fall under
our contractual obligations.

Mr. Patel,
Our Department's recent inspection of the above referenced property on March I Ith and
March 18th, 2015 after sundown revealed that your property is in violation of City of
Saler Zoning Ordinance,Section 4-57. —Illumination and Section 5.1.10- #3 -
Additional Approval by City Engineer(See Regulations below). The main beam located
on the light standard located nearest Essex Street and adjacent to the `billboard' sign is
both aimed towards a public way and reflects onto adjoining premises and streets.
Sec. 4-57. - Illumination.
(a) Flashing lights or simulations of traffic signals or emergency vehicle lights shall not be used on signs.
In addition, red and green colored lights shall not be used when so placed that they could obscure or be a
hazard to the clear vision of traffic control signals viewed from any point on an approaching public way or
if, in the opinion of the building inspector, such light would constitute a hazard.
(b) Spotlights or floodlights shall not be aimed so that any portion of the main beam can be seen
from a public way.
(Code 1973, §3-54)
5.1.10 Additional Approval by City Engineer. After the effective date of this Ordinance, any parcel of
land which is developed as a parking area for ten (10) or more vehicles or as a drive-in business or
automobile, trailer or boat sales or service establishment shall be developed as follows, subject to the
approval of the plans therefore by the City Engineer.
I.Such area, where subject to wheeled traffic, shall be treated with bituminous concrete or equivalent
surfacing and shall have appropriate bumper or wheel guards where needed.
2. A solid wall or fence or compact evergreen screening five (5) feet high shall be erected along all
property lines abutting residential uses.
3. Any light used to illuminate said parking area shall be so arranged as to reflect light away from
adjoining premises and streets.

Based on the plan and site review conducted on March 6,1991 regarding the proposed
retail market, the Health Department makes the following comments in accordance
with 105 CMR 590.000; State Sanitary Code, Chapter X, "Minimum Sanitation
Standards for Food Establishments":
-Walls, floors and ceilings in food preparation, food storage warewashing areas, ,
and all walk-in regrigerators, dressing rooms, toilet rooms shall be constructed
of smooth non-absorbent easily cleanable surfaces. An example is stainless
steel, "glassboard" or equivalent material.
-All equipment and utensils to be constructed of stainless steel or an equivalent
smooth easily cleanable material. Equipment to be installed with adequate
space surrounding it to allow for regular, effective cleaning.
-Accessible wall-hung handwashing sink(s) with liquid soap and paper towel
dispensers to be installed in each food preparation and warewashing area.
Sinks used for food preparation or for warewashing shall not be used for
washing of hands or for any other purpose.
-The installation of exposed horizontal utility service lines and pipes on the
floor is prohibited. Exposed utility service lines and pipes shall. be
installed in a way that does not obstruct or prevent cleaning of the walls,
floors and ceilings.
-A two or three compartment warewashing sink must be installed.. (Industrial,
Self-standing, stainless steel) ,
-All construction must meet the code requirements set forth by the
Building, Fire, and Electrical departments as well as the Health Department.

I'm writing to you in regards to Nicoles Mini Foodstore at the upper
end of Essex Street. " o
Being a .resident of this neighborhood for seyeral.years,_ I take pride
in this area. The problem I an having is with Nicole's Sign. It is so bright
that it is difficult to block the light from the windows of my house, not
only does it cause sleeping difficulties, but casts a bright yellow light
throughout my rooms. This is affecting myself and also my neighbors as well.
To resolve this matter I would like to request that Nicoles shut their sign
off after closing hours.
I would also like to request the removal of a mobile trailor sign that
flashes. I have found that such a sign is illegal in Salem.
I feel with the co--operation of the city and its residents we can resolve
this matter quickly.
