Dear Mr. Cote,
I was asked to take a look at the rear porch of the property at 20 Endiocott Street. The Porches were
constructed of pressure treated wood. The floor joists run perpendicular to the building and are
attached to the rim joist on the building end by metal hangers. The other end was nailed to the outer
rim joist and does not have hangers. The outer rim joist has pulled away from the end of the floor
joists. This is creating the possibility for a structural collapse. You are directed to secure a building
permit and to make the repair within 30 days of this notice. Also, you are directed to not use the porch
until the repairs are complete.

SECTION 3—DESIGN RESPONSIBILITY FOR FIRE PROTECTION SYSTEMS
1. The sprinkler contractor shall provide a registered professional engineer (PE) who
shall produce a complete set of hydraulically calculated sprinkler systems
drawings as required by NFPA-13 and NFPA-13R latest accepted edition. The
sprinkler contractors registered engineer shall make the drawings complete in
every respect and shall show all piping runs, lengths, dimensions, fittings,
elevations call-outs, sprinkler head locations, pipe sizing, equipment layouts,

hydraulic nodes and all appropriate details. The shop drawings shall also show
the existing sprinkler systems and the points of new connection to the existing
piping systems. The sprinkler contractors registered engineer shall provide all
hydraulic calculations to verify the pipe sizing of the sprinkler and standpipe
system. The sprinkler contractors registered engineer shall stamp all sprinkler
drawings and hydraulic calculations and shall submit these to the fire department
and owner's insurance authority for approval and permit. No work fire protection
work shall begin until the sprinkler contractor has obtained the approval for all
sprinkler and standpipe systems from the fire department and the owner's
insurance authority. A new hydrant flow test for the project will be performed by
the sprinkler contractor in order to obtain current flow and pressure data at the
site. The sprinkler contractor shall provide all labor, materials, tools, fees and
coordination necessary for the performing of the new hydrant flow test. The new
hydrant flow test shall be performed in accordance with NFPA-13.
SECTION 4—FIRE PROTECTION SYSTEMS TO BE INSTALLED
The new sprinkler systems will be automatic wet pipe sprinkler systems for 100% of the
building interior spaces. Automatic wet pipe sprinkler systems shall be provided for all
floors.
The new building will be a apartment building with four stories of residential space above
grade and one story of basement space below grade. All building stories are wood frame
construction. Automatic fire sprinkler systems shall be located throughout the four
stories for 100% of the interior spaces as required by applicable codes. Sprinkler heads
for the four residential floors will be located in all rooms as required by NFPA-13R. In
accordance with NFPA-13R small bathrooms and closets that meet the allowable area
exception will not be provided with sprinkler head coverage. Sprinkler heads for the four
residential floors shall be designed and installed in accordance with NFPA-13R. In
accordance with NFPA-13R there will be no fire sprinklers located in any concealed
spaces above ceilings or in unused attics. The sprinkler heads in the basement floor will
be designed in accordance with NFPA-13 and all rooms regardless of size or function
shall be provided with sprinkler head coverage. Due to the fact that there will be no
finished ceilings in the basement level, no concealed spaces above ceilings. The fire
sprinkler systems for the four floors will be automatic wet pipe systems and will be fed
via a single sprinkler riser.

The fire suppression systems described above will be fed via a new fire service water
main brought into the building basement. The underground fire service main shall be
designed and installed in accordance with NFPA-24. A double check valve backflow
preventer shall be provided at the fire service entrance to the building. The backflow
preventer shall be registered with the local water authority by the sprinkler contractor. A
wet system alarm check valve and trim shall be located on the sprinkler riser for the four
floors of the building. A fire department connection will be provided and connected to a
sprinkler riser serving the wet pipe fire suppression systems. In this way the single fire
department connection will have the ability to charge all of the sprinkler systems in the
building. The fire department connection shall be located on the Endicott Street side of
the building and in a location of vehicle access. A water motor gong will be located
adjacent to the fire department connection.
The wet pipe sprinkler systems in residential dwelling areas and connecting corridors
shall be hydraulically calculated in accordance with NFPA-13R using the maximum
sprinkler head within a compartment method. The flow and pressure provided to each
sprinkler head shall be in accordance with the sprinkler head manufacturer listings and
approvals. A 100 GPM hose stream allowance shall be provided. The wet pipe sprinkler
systems in the basement areas shall be hydraulically calculated at an Ordinary Hazard
Occupancy Group 1 with .15 GPM provided for the entire basement floor. A 250 GPM
hose stream allowance will be provided.
All sprinkler heads in dwelling spaces and connecting corridors shall be listed residential
type with temperature ratings in accordance with NFPA-13R. All sprinkler heads in the
basement area shall be listed quick response type with temperature ratings in accordance
with NFPA-13.
All sprinkler systems will be designed, tested, flushed and installed in accordance with
the applicable sections of the Massachusetts State Building Code 61h edition and NFPA-
13, NFPA-13R and NFPA-24 latest accepted editions as well as the regulations and
guidelines of the local authority. All design, installation and testing work shall be
performed by a sprinkler contractor licensed and experienced in the State of
Massachusetts in the design, installation and testing of sprinkler systems. All sprinkler
systems material and equipment shall be UL listed and or FM approved for fire protection
service and shall be rated at 175 psi working pressure. All sprinkler systems shall be
seismically braced in accordance with the Massachusetts State Building Code 6`h edition,
and NFPA-13.

SECTION 5—FEATURES USED IN THE DESIGN METHODOLOGY
FIRE ALARM SYSTEM
1. The fire alarm system shall be an addressable system that meets UL, the requirements
for NFPA and the Salem Fire Department Bylaws. The devices shall be installed in
accordance with the floor plan layouts. The main fire alarm control panel shall be
located in the lower level electric room. There shall be an annunciator located at the
main entrance to the building. The remainder of the building will be provided with
required pull stations, horn strobes and strobe only units as required by NFPA, ADA
and Massachusetts State Building Code.
2. New Audio/visual alarms shall be utilized throughout the facility and the coverage
shall meet the requirements of NFPA and ADA. New Visual alarms shall be utilized
in small areas such as bathrooms and corridors were ample audio alarm is present.
3. Batteries shall provide for 24 hours of operation followed by a 10-minute ring down.
Battery calculations shall be submitted by the EC with the cut sheets and drawings to
the fire department for review and approved.
4. Emergency lighting shall be provided throughout the facility and consists of self-
contained exit signs with battery back up and fluorescent fixtures with integral battery
ballasts and test stations as well as a few emergency battery units.
5. New tamper, flow and pressure switches are being added to accommodate the new
sprinkler system being added. The tamper and flow switches shall be connected to
the Fire Alarm Control Panel via address modules.
SECTION 6—TESTING CRITERIA
1. All Testing of the sprinkler systems devices, piping, heads and valves, shall be
performed by a licensed sprinkler contractor, experienced in the installation and
testing of fire sprinkler systems in the state of Massachusetts. This sprinkler
contractor shall provide all coordination of the sprinkler tests with the fire
department. All sprinkler systems tests shall be witnessed by the fire department,
owners representative, owners insurance authority. The sprinkler contractor shall
give notification to the local fire department, owners representative, owners

insurance authority and all other authorities, prior to the conducting of any
sprinkler system tests.
2. All sprinkler systems testing shall be performed in strict accordance with NFPA-
13,NFPA-13R,NFPA-24 and NFPA-25, latest accepted editions.
3. The sprinkler contractor shall provide all equipment, water, pumps, labor, etc.,
necessary to conduct all testing required by NFPA 13, 13R, 24 and 25, and shall
obtain all required approvals from the fire department, building code official,
owners insurance authority, and all other authorities upon completion of the
sprinkler systems testing. All dates and times of the sprinkler systems testing
shall be coordinated by the sprinkler contractor with the fire department, owners
representative, and the owners insurance authority.
SECTION 7—EQUIPMENT AND TOOLS
1. The sprinkler contractor shall have at the site, at the time of sprinkler installation
and testing, and readily available for use and inspections, all tools, equipment,
permits, hoists, scaffolding, piping, fittings, valves and all required tools and
equipment necessary for the complete installation of all fire protection systems.
2. The sprinkler contractor shall provide all gauges, valves, hoses, smoke, etc.,
required for an operational test of the installed fire sprinkler system. Remote test
and drain stations shall be provided for system performance testing.
3. The sprinkler contractor shall have, at the site, the manufacturers instructions and
nameplate data for all fire sprinkler equipment, and devices being used in the
sprinkler systems installation.
SECTION 8—APPROVAL REQUIREMENTS
1. Upon successful inspection and testing of all fire sprinkler systems, the sprinkler
contractor shall obtain written approval of the completed fire sprinkler system
from the fire department, owners insurance authority, and all other code officials.
Copies of all written approvals shall be turned over to the owner representative.
2. If a code official should find cause to reject any portion of the fire sprinkler
system installation or testing, the sprinkler contractor shall be responsible for
correcting the problem in a timely manner. Upon completion of all corrective
measures undertaken by the sprinkler contractor, a new test and inspection shall
be again coordinated by the sprinkler contractor, and witnessed by the fire
department and all other authorities as previously stated.

3. The sprinkler contractor shall submit stamped and signed sprinkler design
drawings and hydraulic calculations and obtain all required permits prior to the
installation of any fire sprinkler systems. The sprinkler plans shall be in
accordance with the Massachusetts State Building Code, 61h edition, chapter 9, as
well as the requirements in NFPA-13„NFPA-13R latest accepted editions.
4. The sprinkler contractor shall obtain all document of acceptance as required by the
Massachusetts State Building Code, 61h edition, chapter 903.4, and shall submit all
such documents as required, to the owners representative and building code
officials.
