Dear Ms. Colon,
The Salem Board of Health is contacting you by mail because this office has tried to
contact you by phone multiple times without success. A re-inspection of your apartment
is required in accordance with Chapter 111, Sections 127A and 127B of the
Massachusetts General Laws, 105 CMR 400.000: State Sanitary Code, Chapter 1,
General Administrative Procedures and 105 CMR 410.000: State Sanitary Code,
Chapter II: Minimum Standards of Fitness for Human Habitation.
Please contact the Salem Board of Health at 978-741-1800 to schedule an appointment
for re-inspection.
Failure to comply with this order for re-inspection may result in either the case being
dropped, or a complaint being sought against you in Salem Housing Court.
