SYSTEMS.
To Whom It May Concern:
The Fire Prevention Bureau has no recent records of any inspection,testing,or
maintenance of your fire alarm and/or sprinkler systems. These fire detection and fire
suppression systems are to be tested annually by a licensed, qualified company.
All fire detection and suppression systems are to be inspected,tested, and maintained in
accordance with the State Building Code, 780 CMR Chapter 9:Fire Protection
Systems,NFPA 25.standard for the Inspection, Testing, and Maintenance of Water-
Based Fire Protection Systems,and NFPA 72:National Fire Alarm Code.
Inspection,testing, and maintenance of these systems is the responsibility of
the owner of the fire alarm and/or sprinkler system.
In addition,commercial kitchen hood and duct systems are to be inspected, tested, and
maintained in accordance with NFPA 96:standard for Ventilation Control and Fire
Protection of Commercial Cooking Operations.

A copy of the Fire Prevention Bureau's hood and duct notification letter has been
enclosed with this letter where applicable.
Inspection, testing, and maintenance of the kitchen system is the system
owner's responsibility.
Proper documentation of a schedule of system compliance will be a
requirement for renewal of an annual food service permit and an
annual liquor license.
Please forward to this office documentation of any recent work or testing performed on
such systems so that we can update your file.

The Salem Fire Prevention Bureau requires that all hood and dud systems are to be cleaned
from the cooking area (hood) in the kitchen, all the way to the extreme end of the duct. THIS
CLEANING WILL OCCUR ON A QUARTERLY BASIS.
A certificate showing the name of the company, the person,.and the date of the cleaning shall be
posted near the hood area. The certificate will indicate what work wis perfonned, and will also
indicate areas not cleaned.
ALL FIXED FIRE EXTINGUISHING SYSTEMS ARE TO BE TESTED AND CHECKED SEMI-
ANNUALLY. THIS TEST WILL ALSO INCLUDE A TEST OF THE INTERIOR FIRE ALARM
SYSTEM.
The cleaning company and the fixed extinguishing service company shall file within (5) five days
after the work is performed, a report with the Salem Fire Prevention Bureau of all their activities
relative to the above listed systems. The report shall also list any and all deficiences of the
systems and the remedial action to be taken. Failure to adhere to this public safety requirement
will result in the shut down of the systems. Prior to resuming operations an inspection will be
conducted by this office.
