Construction supervisor,Rod Rivera Lic#082273 Tax ID#2C-0309650 This agreement is made this 8/12/2024 by and between Rod Rivera(hereinafter"Contractor") Maria R.Nunez and Joselito Collazo 91 TREMONT STREET Salem,Ma 01970(Hereinafter "Owner'), Scope of Work The owner/agent hereby engages the contractor and designer to provide all necessary supervision, labor,and materials for the following tasks: 1.Removal and Relocation of Interior Staircase: o The existing interior staircase currently located within the bathroom shall be fully removed. o The contractor is responsible for all demolition work,Including the safe removal of any structural elements and finishing as required. o The staircase will be relocated to the proposed bump-out area,which involves the construction of a new structural extension as per the approved design plans. o All associated framing,flooring,and finishing work required to Integrate the relocated staircase into the bump-out will be completed in accordance with building codes. 2.Construction of a Dormer: o A new dormer shall be constructed above the staircase area to increase headroom and provide additional space in the attic, o The dormer construction will include framing,roofing,siding,and window installation as per the design specifications. a The contractor will ensure proper integration with the existing roof structure,including flashing,insulation,and weatherproofing. a Interior finishing of the dormer area will Include drywall,painting,and any necessary trim work to ensure a seamless connection with the existing interior spaces. The contractor and designer are responsible for ensuring that all work is carried out in compliance with local building codes and regulations,and for coordinating with any necessary subcontractors to complete the project as specified. The homeowner is responsible for purchasing all materials for the trades they have hired,except for subcontractors agreed to purchase some materials on their own.Rod Rivem will not be responsible for purchasing any materials.No contractor or subcontractor is permitted to work on the property unless Rod Rivera has agreed to and accepted the conditions of their perfommnce and schedule to avoid Pity liabilities.Rod Rivera is not responsible Ibr subcontractors hired by Lite homeowner;all tradespersons must have current worker's compensation and liability insurance,presented to Rod Rivera,unit must be up-to-date. General Guarantee The Contractor shall remedy any defect due to faulty material or workmanship and pay for any damage to other work resulting therefrom which shall appear within the period of one year from final payment Further, the Contractor will furnish Owner with all manufacturers and supplier's written guarantees and warranties covering materials and equipment furnished under this Contract. Permits and Codes The Owner shall obtain all necessary building permits, including those required by the Massachusetts and section r105 in 780 CMR, 18C The Contractor will secure at his/her own expense any other necessary permits and licenses required to do the work and will comply with all building and code regulations and ordinances whether or not covered by the specifications and drawings for the work t Work Performance ,! 1 The Contractor shall protect all work adjacent to the Contract site from any damage resulting from the work of the Contractor and shall repair or replace any damaged work at his/her own expense 2) The Contractor shall replace and put in good condition any existing conditions damaged in carrying out the contract 3) The Contractor shall take all precautions to protect persons from injury and unnecessary interference or inconvenience It 41 The Contractor shall conduct his activities in a business like manner and adhere to the reasonable wishes of the Owner in relation to his working schedule Condition of Premises The Contractor agrees to keep the premises clean and orderly and to remove all debris as needed during the hours of work in order to maintain work conditions which do not cause health or safety hazards Use of Utilities The Owner shall permit the Contractor to use,at no cost, power and water necessary to the carrying out and con pleGon of the work 9 Use of On Site Loam Owner Agrees as part of this contract to allow the Contractor use of onsite loam to the degree available as necessary for the completion of the project Inspection The Owner shall have the right to inspect all work performed under this contract As well it shall be a condition of!his contract that all work that needs to be inspected of tested and certified by the engineer as a condrfion of the Dept of Environmental Conservation Permit.
