Dear Sir/Madam:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum
Standards of Fitness for Human Habitation, a recent re-inspection was conducted of your
property located at 12 Pope Street Unit A105, conducted by Jeffrey Barosy, Sanitarian of the
Salem Board of Health on January 10, 2025. At that time the following items were still
outstanding:
The occupant has stated the management is having pest control visit her dwelling unit
regularly for mice (see report for further information). The inspector will conduct another
re-inspection to confirm the dwelling unit no longer has mice.
Please continue to exterminate mice in the dwelling unit until they are no longer a public health
nuisance.
All other violations noted in the inspection report of October 30, 2024, have been
corrected.

Observations:Two dead cockroaches were observed in the cupboards beneath the
kitchen sink. Have a licensed pest control service exterminate the cockroaches in the
dwelling unit. Extermination is to continue until cockroaches are no longer a public
health nuisance.
Code Description: Owners and occupants are responsible for the control and
elimination of pests as follows: (B)The owner of a residence containing two or
more dwelling units, including a homeless shelter or a rooming house, shall
maintain it and its premises free from infestation and shall be responsible for pest
elimination.

Occupant informed the inspector pest control visits the dwelling unit often and usually removes a captured mouse on each visit.
Occupant stated she has seen and heard mice less often since the initial inspection, indicating an improvement.At the time of
the re-inspection the inspector did not observe signs of mice.
The occupant also informed the inspector she has not seen any cockroaches since the initial inspection. The inspector did not
observe evidence of cockroach presence at the time of re-inspection.
Occupant informed the inspector that for a few weeks the water was intermittently discolored, but has been clear for the last two
weeks.The inspector did not observe rusty or dirty water from the sink faucets or the bathtub faucet at the time of re-inspection.
The inspector plans to conduct another re-inspection on another date to check the status of the mice presence in the dwelling
unit.
