Dear Sir:
On January 20, 1976 you were granted a permit X611 to erect a
single family residence at the above referenced address. Your
permit was issued with the stipulation that you would "build
according to the following specifications:-" as outlined in
your application.
Upon inspection of this project, I discovered the building is
not as shown on the plans for which said permit was issued. This
is a violation of the Massachusetts State Building Code, Section 115.2,
"Compliance With Permit."
You are hereby notified to cease all work on this property and
that the building shall not be occupied until conditions of the
permit are satisfied. Further, be advised that permit #11 has been
revoked.

Dear Sir:
My inspection at the referenced property shows that the residence
built thereon does not conform to plans filed with permit #11
issued January 20, 1976. You were also informed that once the
foundation was installed, a certified plot plan should be filed
with this office. This you have failed to do.
You are hereby informed that your General Contractor's License #013,
issued January 20, 1976 is hereby revoked, for failure to conform
with conditions of said permit, which constitutes a violation of
the Massachusetts State Building Code, Section 115.2.
