Records at this office indicate that you have failed to file Form 4V82,
Certificate of Completion; and/or arranged for a test of fire protection
equipment, installed under provisions of the General Laws of the Commonwealth
of Massachusetts, at the above location.
Application to install was made April 7, 1976
It appears that sufficient time has elapsed since the application date
and/or occupancy has been established. Before occupancy a test was
to be conducted by appointment.
You are therefor in violation of the General Laws of the Commonwealth.
