Dear Sir/Madam:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum
Standards of Fitness for Human Habitation, a recent re-inspection was conducted of your
property located at 40 Ward Street Unit 1 L, conducted by Jeffrey Barosy, Sanitarian of the
Salem Board of Health on August 12, 2024. At that time the following items were still
outstanding:
Item# 6: There is still a cockroach infestation in this unit. Cockroaches were found in
the kitchen, living room, bedrooms, and bathroom.
Because this has been an issue that has persisted for a few years at this point, starting
from the date of the re-inspection, the owner of this property will receive monetary fines
for every day this violation continues. Failure to correct this violation in a timely manner
will result in this matter being taken to housing court. Please correct the outstanding
violations and contact this office to schedule a final re-inspection of this apartment.
Thank you for your cooperation in this matter:
