Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection,contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Violation (s)_ Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 24 Hours .550 1. Mice droppings were found on both levels of the dwelling unit in various .630 (17) locations that include the bedroom, the bathroom attached to the bedroom, the kitchen, and the living room closet. The owner is to exterminate mice in the dwelling unit and all adjacent walls. Any holes leading into the dwelling unit must be plugged to prevent re-entry from mice. Extermination is to continue as needed until mice no longer resent a public health nuisance in the dwelling unit. 30 Days .320 2. There is a lamp hanging from the ceiling near the rear dwelling unit entrance with a power cord going through a wall and plugging into a ceiling light in the rear common hallway. Unplug and remove this lamp. Temporary wire of this kind is not allowed. 24 Hours .500 3. The dining room floor has nails protruding out of the wooden floorboards .630 (12) and metal trim around the vinyl floor with corners not flush to the floor, potentially cutting or stabbing objects that pass over them. Repair the floorboards so they are smooth with no protruding nails and no splinters and repair the metal trim, so they are flush to the floor. 24 Hours .500 4. The atrium on the first floor in the space between the living room, kitchen, .630 (12) and dining room has evidence of water leakage on its ceiling with discolored paint. Find the source of the water leak and repair it. Scrape off discolored paint and repaint effected area N/A N/A 5. NOTE: There is evidence of water leakage coming from the skylight windows on the second floor with water-stained paint on the walls beneath them. Since it was not raining at the time of inspection the inspector could not determine if the windows were allowing water in while closed or if the stains were from the windows being open while it was raining. The inspector advises the owner to have these windows inspected to determine if they properly keep water from infiltrating the dwelling unit. If they do not, have them repair. N/A N/A 6. The owner has 30 DAYS to correct all violations listed in this report. The owner must start making good-faith effort (reporting to the Salem Board of Health what actions will be taken in writing, hiring contractors that may be needed, or starting necessary work on his/her own)within the time frame noted beside each violation. If an extension beyond 30 days is needed to make corrections, the owner must request a hearing before the Board of Health within 7 DAYS of receipt of this report and acquire a::.: roval for the extension.
