Terms and Conditions
i) This agreement does not guarantee past,present,or future damage to the building or contents,or replacement
thereof. This agreement does not provide for control of termites, power post beetles, bedbugs,trapping of bats,
squirrels, raccoon etc. But are available at additional cost if ever needed.The Company's liability hereunder is
limited only to retreatment within the warranty or contract period.
z) In case of non-payment or default by the purchaser, NW Pest Control Inc. has the right to terminate this agreement
and reasonable attorney's fees and costs of collection shall be paid by the customer whether the suit is filed or not.
In addition,interest at the highest legal rate will be assessed for the period of delinquency.
3) Large rodent boxes will be additional if ever needed.
4) Monthly unit allowance is "use them or lose them", no units will carry over;in-between services will be at a charge.
5) Additional unit overage charge:$35/onsite,$95 special visit(must be more than 3, 15t unit charged @$95)
6) It is the client's responsibility to post all notice on tenant doors 24-48 hours prior to treatment.
7) BurrowRx, Microinjection,and Dust treatments are at additional cost.
8) Work requiring a 20ft or above ladder will be at additional cost.
9) If service is cancelled under 24 hrs before appointment there will be a$95.00 trip charge assessed to the account.
ia) This agreement will be for an initial period of one year;the contract may be canceled,or the frequency can be
adjusted by either party by given a thirty(30)day written notice after the first year.
ii) Quotation subject to change after 30 days.
