Chapter II: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 116
Lafayette Street,conducted by Jeffrey Barosy,Sanitarian on May 29,2024 at 9:30am.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.

of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following
were noted:
30 Days .235 1. Unit 316's sink drains slowly. Repair sink so its drain is free of
obstructions, and it drains water adequately.
.540 2. Unit 316's right-side window lacks a screen. Provide a screen of
30 Days adequate size for this window.
.500 3. Unit 316 has two ceiling tiles that are bowing, indicating they are
30 Days ready to fall from the ceiling. Repair or replace these ceiling tiles so
they are set properly within the ceiling tile mounts without bending or
breaking
.235 4. Unit 313's bathroom ceiling fan does not work. Repair this ceiling fan
30 Days so it adequately vents moisture out of the bathroom.
.540 5. Unit 313's right-side window lacks a screen. Provide a screen of
30 Days adequate size for this window.

30 Days for this shower stall that is smooth, easily cleanable, and properly
affixed to the wall.

.235 16. Unit 118's sink has a broken faucet handle. Repair this sink's faucet
30 Days handle so it can turn on hot and cold water as designed.

24 Hours .630 (9) allowing the door to open after being pushed close. Repair or replace
this doorknob so it opens and closes as designed and door can
remain closed when pushed closed.
.110 18. The shared first floor bathroom near unit 116 has a bathtub with a
30 Days chipping and peeling surface interior surface. Refinish the surface of
_ this bathtub so it is smooth, intact, and easily cleanable.
.110 19. The shared second floor bathroom near unit 216 has a bathtub with a
30 Days chipping and peeling surface interior surface. Refinish the surface of
_ this bathtub so it is smooth, intact, and easily cleanable.

and repaint effected areas of ceiling.

beside each violation. If an extension beyond 30 days is
needed to make corrections, the owner must request a hearing
before the Board of Health within 7 DAYS of receipt of this
report and acquire approval for the extension.
