In accordance with Chapter 111, Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000;
State Sanitary Code,Chapter 1:General Administrative Procedures and 105 CMR 410.000: State Sanitary Code,
Chapter II: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 6
Lemon Street Unit 2 permitted by occupant Rose Lopez conducted by Jeffrey Barosy, Sanitarian on May 7,2024
at 2:00pm.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted: _
24 Hours 270 1. The door for the rear entrance has a deadbolt that requires keys for
.630 (9) both sides and a doorknob lock that does not work. Repair this door's
locks so the deadbolt has a turn button on the side facing into the
dwelling unit and the lock on the doorknob locks and unlocks as
designed.
.235 2. The switch for the ceiling light and fan fixture in the kitchen does not
30 Days work. Repair this switch so it works as designed and power to the
ceiling light and fan fixture in the kitchen can be turned on and off.
.170 3. The vent above the stove vents into the kitchen. Repair this vent so it
24 Hours .630 (2) vents to the outdoors.
.500 4. The kitchen closet's walls have chipping and peeling paint. Remove
30 Days chipping and peeling paint and repaint effected areas.
.150 5. The hot water at the kitchen sink has a temperature of 133.3°F.
30 Days Reduce the hot water temperature so it is between 110°F and 130°F.
.235 6. The faucet head for the kitchen sink is missing. Repair or replace the
30 Days kitchen sink's faucet head.
.500 7. The bathroom ceiling and part of its walls show signs of water
24 Hours .630 (12) leakage with discoloration and mold growth. Find the source of the
water leakage and repair it. Remove mold growth from ceiling and
walls and repaint effected surfaces.
.500 8. The bathroom walls and the window's border have chipping and
30 Days peeling paint. Remove chipping and peeling paint and repaint
effected areas.
.235 9. One of the wall mounted switches for one of the bathroom's ceiling
30 Days lights does not work, preventing one of the ceiling lights from turning
on and off. Repair this switch so the bathroom's ceiling light turns on
and off as desi ned.
.235 10. The bathroom sink has water leaking out of its drainage pipe. Repair
30 Days this sink's drainage pipe so it no longer leaks water.
.500 11. The bedroom ceiling and the corner of the wall that juts out near the
30 Days bedroom door both have chipping and peeling paint. Remove
chipping and peeling paint and repaint effected areas.
.235 12. The wall mounted switch for the bedroom's ceiling light does not
30 Days work. Repair this switch so the bedroom's ceiling light turns on and off
as designed.
.500 13. The bedroom ceiling shows signs of water leakage with discolored
24 Hours .630 (12) paint in one corner of the bedroom. Find the source of the water
_ leakage and fix it. Remove paint and repaint effected areas.
.235 14. The living room's ceiling light and fan fixture is hanging from an
30 Days electrical wire. Repair this fixture so it is properly affixed to the other
fixture parts and ceiling as designed.
.235 15. The wall mounted switch for the living room's ceiling light and fan
30 Days fixture does not work. Repair this switch so it works as designed and
power to the ceiling light and fan fixture in the living room can be
turned on and off.
.235 16. The thermostat on the living room wall is broken. Repair or replace
30 Days this thermostat so that it turns the heat on and off as designed.
.260 17. The stairs for one of the unit's egresses is blocked by three wooden
24 Hours .630 (8) bars across the handrails. Remove these wooden bars so this egress
is not obstructed.
.270 18. The door for one of the dwelling unit's egresses has a missing
24 Hours .630 (9) doorknob and a misaligned deadbolt lock. Supply a doorknob for this
door and repair the deadbolt lock.
.570 19. There is construction debris collecting on the ground at the rear of the
30 Days property. Have this construction debris disposed of in a sanitary
manner.
N/A 20. NOTE: The occupant blocked one of the egresses with furniture from
N/A within the dwelling unit. The inspector ordered the occupant to
remove the furniture and to keep all egresses free from obstruction.
N/A 21. NOTE: There is a pipe in the living room that stretches through a wall
N/A between the living room closet and the chimney. The inspector is not
a licensed plumber and could not confirm if this is a plumbing issue.
The inspector will consult the City's Plumbing Inspector to see if
action needs to be taken.
N/A 22. NOTE: The occupant informed the inspector the spotlight at the top of
N/A the stairs leading to the rear entrance for the dwelling unit does not
work. The inspector could not confirm this at the time of inspection.
The inspector advises the owner to investigate this complaint and
repair the light if it does not work. _
N/A 23. NOTE: The occupant informed the inspector the dwelling unit's heat
N/A is controlled by the first-floor dwelling unit and it is not always turned
on during the heating season. The inspector could not observe this at
the time of inspection. The inspector advises the owner to make sure
that during the heating season, September 15th to May 31 st, heat is
always provided to this dwellin::: unit between the temperatures 68°F_]
and 780F between 7:OOam and 11:00pm, and 640F and 780F between
11:01 pm and 6:59am.
N/A 24. NOTE: The occupant informed the inspector the circuit breakers for
N/A her dwelling unit also controls the power in the attic. The inspector
could not confirm this at the time of inspection. The inspector advises
the owner to have electrical cross metering fixed if cross metering
exists. The inspector will inform the City's Building Department about
_ the possibility illegal apartment in the attic.
N/A 25. The owner has 30 DAYS to correct all violations listed in this report.
N/A The owner must start making good-faith effort(reporting to the Salem
Board of Health what actions will be taken in writing, hiring
contractors that may be needed, or starting necessary work on
his/her own)within the time frame noted beside each violation. If an
extension beyond 30 days is needed to make corrections, the owner
must request a hearing before the Board of Health within 7 DAYS of
receipt of this report and acquire api-roval for the extension.
One or more of the above violations may endanger or materially impair the health, safety and well-being of the
occupant(s)
