In accordance with Chapter 111, Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000;
State Sanitary Code,Chapter 1: General Administrative Procedures and 105 CMR 410.000: State Sanitary Code,
Chapter II: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 73
Harbor Street as part of an annual rooming house inspection conducted by Jeffrey Barosy, Sanitarian on April 17,
2024 at 9:30am.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies
available to them as outlined in the enclosed inspection report form.

An annual rooming house inspection was conducted in accordance with Article II
of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following
were noted:
30 Days .530 1. Unit 203's left-side window has a bottom sash that is difficult to open.
Repair this window sash so it opens and closes with ease.
.540 2. Unit 203's right-side window lacks a screen. Provide a screen for this
30 Days window that covers the entire openable portion of the window.
Screens need to be in place between April 1st and October 3111 during
the year.
.530 3. Unit 209's window has a bottom sash that does not stay up under its
30 Days own power when lifted. Repair this window so it can stay up under its
own power when the window is opened.
.270 4. Unit 206's left-side window is missing lock keepers. Repair this
24 Hours .630 (9) window so it has properly installed lock keepers and window can lock
and unlock with ease.
.500 5. Unit 310 door has a gap at the top between it and the doorframe that
30 Days is large enough for people to see into the rooming unit from the
hallway. Repair or replace this door so it fits in the doorframe in a
manner where there are no gaps large enough for people to see into
the rooming unit. Doors to rooming units need to provide privacy to
occupants.
.270 6. Unit 303's right-side window has no lock keepers. Repair this window
24 Hours .630 (9) so it has properly installed lock keepers and window can lock and
unlock with ease.
r .530 7. Unit 303's right-side window has a crack in the glass in its top sash.
30 Days Repair the glass in this window's top sash so it no longer has a crack.
.270 8. Unit 302's right-side window has no lock keepers. Repair this window
24 Hours .630 (9) so it has properly installed lock keepers and window can lock and
unlock with ease.
.530 9. Unit 302's windows' sashes sag. Repair these windows so their
30 Days sashes no longer sag and can open and close with ease.
.270 10. Unit 305's left-side window has no lock keepers. Repair this window
24 Hours .630 (9) so it has properly installed lock keepers and window can lock and
unlock with ease.
.270 11. The third-floor bathroom's window does not have a lock. Install a lock
30 Days .630 (9) and lock keeper on to this window so it can lock and unlock with
ease.
N/A 12. The owner has 30 DAYS to correct all violations listed in this report.
N/A The owner must start making good-faith effort(reporting to the Salem
Board of Health what actions will be taken in writing, hiring
contractors that may be needed, or starting necessary work on
his/her own)within the time frame noted beside each violation. If an
extension beyond 30 days is needed to make corrections, the owner
must request a hearing before the Board of Health within 7 DAYS of
receipt of this reiPort and acquire approval for the extension.
