Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection,contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order. Violation(s) An annual rooming house inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 30 Days .500 1. In 4 Howard Street Unit 1 there is duct tape covering up a hole.in a bathroom ceiling tile. Repair or replace this ceiling tile so there is no longer a hole. (NOTE: The occupant reported hearing and seeing an animal in the ceiling. The inspector did not observe this at the time of inspection. Inspector advises owner to investigate this matter and exterminate any animal that may be in the ceiling.) .330 2. 4 Howard Street Unit 5 does not have a carbon monoxide detector. 24 Hours .630 (15) Provide a carbon monoxide detector for this rooming unit. .235 3. 2 Howard Street Unit 4's stove's back-right burner is not working. 30 Days Repair this burner so it turns on and off with ease. .530 4. 2 Howard Street Unit 4's right-side window has a bottom sash that 30 Days falls when lifted. Repair this window's sash so it can stay up under its own power and can open and close with ease. .500 5. 2 Howard Street Unit 2's kitchen ceiling shows signs of water leakage 24 Hours .630 (12) with cracks and discolored paint. Find the source of the water leakage and repair it. Scrape off paint in affected area of ceiling and repaint. .500 6. One set of basement stairs has a drainage pipe going through one of 30 Days the steps. Repair stairs so there is no longer a pipe or any object going through it that compromises its structural integrity. .500 7. In one section of the basement there is water damage with mold 30 Days growth on a wooden wall. Find the source of the water and repair it. L_ Remove mold from wooden wall. .500 8. There is a section of the roof that has protruding pipes with plastic 24 Hours .630 (12) tarp around their bases to keep out water. Repair this section of the roof so that the pipes no longer need plastic tarp to keep water from _ infiltrating _ N/A 9. NOTE: 4 Howard Street Unit 2 has a separate kitchen and bathroom. N/A N/A 10. The owner has 30 DAYS to correct all violations listed in this report. N/A The owner must start making good-faith effort(reporting to the Salem Board of Health what actions will be taken in writing, hiring contractors that may be needed, or starting necessary work on his/her own) within the time frame noted beside each violation.
