Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection,contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. Violation(s) Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: �_24 Hours .500 1. The bedroom adjacent to the bathroom has a part of its wooden floor that is splintering. Repair this portion of this bedroom's floor so it is no longer splitting and has a smooth and easily cleanable surface. .500 2. The bedroom adjacent to the bathroom has holes in the floor around 30 Days the baseboard heater indicative of mice. Repair the holes in the floor and seal off gaps to prevent mice entry. .500 3. The kitchen ceiling above the kitchen sink has a hole from a water 24 Hours .630 (12) leak. Find the source of the water leak and repair it. Repair the hole in the kitchen ceiling so the kitchen ceiling is restored to the condition it was in before the water leak. _ .550 4. Mice droppings were found on the floor in the kitchen behind the 24 Hours .630 (17) stove and in a drawer in the bedroom directly across from the bathroom when traveling through the kitchen. The owner is to exterminate mice in the dwelling unit and all adjacent walls. Any holes leading into the dwelling unit must be plugged to prevent re-entry from mice. Extermination is to continue as needed until mice no longer present a public health nuisance in the dwelling unit. .100 5. The kitchen cabinet above the stove has a broken frame. Repair this 30 Days cabinet's frame. .100 6. The kitchen cabinets above the refrigerator are coming loose from the 30 Days wall. Repair cabinets so they are properly affixed to the wall. N/A 7. NOTE: Occupant informed the inspector that the shower stall in the N/A bathroom has a leaking ceiling. At the time of inspection, the inspector did not see evidence of water leakage in the bathroom ceiling above the shower stall. The inspector advises the owner to investigate water leakage in the bathroom ceiling, as there is a water leak in the adjacent kitchen. N/A 8. The owner has 30 DAYS to correct all violations listed in this report. N/A The owner must start making good-faith effort(reporting to the Salem Board of Health what actions will be taken in writing, hiring contractors that may be needed, or starting necessary work on his/her own) within the time frame noted beside each violation. If an extension beyond 30 days is needed to make corrections, Owner must request a hearing before the Board of Health within 7 DAYS of receipt of this report and acquire approval for the extension.
