QT 4 Balcomb St November 8, 2019 Paul & Cheri Lee McGoldrick 4 Balcomb Street Salem, MA. 01970 Dear Sir/Madam: The Board of Health has received notification that you are in violation of one or more of t he following Board of Health regulations as indicated: Property located at: 4 Balcomb Street in Salem, Massachusetts. _____ Trash placed at curbside before 6:00pm the evening before collection. (BOH Regulation 7, Sec 5.1.1) During the week of a holiday, trash pick-up is one day later than usual. __X___ Trash bags, containers or any other item not taken for municipal pick up and left at curbside longer than 24 hours. Trash, yard waste or any other item not taken for municipal pickup on your scheduled day, must be removed/ pulled back from curbside the same day as the collection. (BOH Regulation 7, Sec 5.1.3) _____ More than one bulk waste item placed at curbside. (BOH Regulation 7, Sec 5.4) _____ Cardboard not bundled properly. Cardboard must be flattened in lengths no more than 3 feet and bundled with weights of no more than 50 pounds, or placed in paper bags or appropriate recycling containers. (BOH Regulation 7, Sec 5.5) _____ Yard waste placed at curbside on days other than Salem’s scheduled pick up dates. _____ Appliance “white goods” placed at curbside for municipal pick-up (BOH Regulation 7, Sec 5.8) Remove from curb & contact Waste Management at 1-800-972- 4545 for instructions on free pick-up. _____ Placement of construction or demolition waste at curbside for municipal pick -up (BOH Regulation 7, Sec 3.4) Contact private disposal company or the Salem Transfer Station at (978) 745-0226 for drop off times. _____ Placement of Hazardous Materials at curbside: Type of Material_______________________ (BOH Regulation 7, Sec 3.6) _____ Placement of computer or television monitors at curbside for municipal collection. Remove from curb & contact private disposal company or contact Salem Department of Public Services at (978) 744-3302 for sticker program information. You are ordered to correct the above violation(s) within twenty-four (24) hours. Should you be aggrieved by this order, you have the right to request a hearing before the Board of Health. A request for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this order. At said hearing, you will be given an opportunity to be heard and to present witness and documentary evidence as to why this order should be modifi ed or withdrawn. You may be represented by an attorney. Please also be informed that you have the right to inspect and obtain copies of all relevant inspection or investigation reports, orders, and other documentary information in the possession of this Board, and that any adverse party has the right to be present at the hearing. Continuation of this violation or its recurrence will result in the issuance of a $25 citation for each day the violation exi sts.
