Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
24 Hours .500 There is air coming in through the windows and black scum growing around
them. Owner must repair windows so that they are weather tight and free from
chronic dampness. Owner has 24 hours to correct violation.
24 Hours .550 (B) Cockroaches found behind refrigerator. Owner is to hire a professionally licensed
exterminator. Owner must send extermination report to the Salem Board of
Health after the extermination has taken place. Exterminator is to return as
often as needed to rid the building of cockroaches. Owner has 24 hours to
correct violation.
5 Days .351 Screens in windows are torn. Repair screens so that they are in good working
conditions. Owner has 5 days to correct violation.
5 Days .500 Bathroom window is nailed shut. All windows inside unit must be able to open
and close. Fix and repair window so that its working as intended. Owner has 5
days to correct violation.
One or more of the above violations may endanger or materially impair the health, safety and well-being of the
occupant(s)
