Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
7 Days .551 Window located near main entrance and back entrance are both missing a
screen. Provide a screen for both windows. Owner has 7 days to correct
violation.
7 Days .500 Kitchen ceiling is slightly bulging indicating that there may be a possible leak.
Find leak and repair, and repair and repaint ceiling. Owner has 7 days to correct
violation
24 Hours .500 There is a hole behind kitchen stove. Patch hole so that the wall/ floor is free
from holes or cracks. Owner has 24 hours to correct violation
24 Hours .482 Carbon monoxide needed close to the main bedroom. Install carbon monoxide.
Owner has 24 hours to correct violation
7 Days .500 You can see light coming from the basement in the girls bedroom through the
floors. Seal cracks between floorboard. Owner has 7 days to correct violation
7 Days .500 The siding on the house needs to be finished and repaired. Owner has 7 days to
correct violation.
7 Days .600 There are multiple items in the backyard covered with a tarp that needs to be
removed. Property must be maintained in a clean, sanitary manner and kept free
of trash and other debris as this may attract rodents. Owner has 7 days to
correct violation
24 Hours .550 (B) Mice droppings found in main bedroom. Owner is to hire a professionally
licensed exterminator. Owner must send extermination report to the Salem
Board of Health after the extermination has taken place. Exterminator is to
return as often as needed to rid the building of mice. Owner has 24 hours to
correct violation. Owner has 24 hours to correct violation.
