Upon Inspection the following were noted: 24 Hours .500 Bathroom ceiling has a hole and water damage above tub, as well as chipping paint on window sill. Repair hole and water damage, and scrape and repaint window sill. Owner has 24 hours to correct violation. 7 Days .235 (A) Bathroom toilet tank cover is cracked. Repair or replace cover so that is in good working condition. Owner has 7 days to correct violation. 7 Days .500 Bathroom wall near the bottom of the toilet is missing some tiles. Repair or replace tiles so that they are in good working condition. Owner has 7 days to correct violation. 24 Hours .500 Child bedroom has water stains on ceiling indicating a leak. Repair and paint ceiling so that its free of leaks. Owner has 24 hours to correct violation. 7 Days .500 Kitchen wall above cabinets is unfinished. Repair and finish wall so that its in good condition. Owner has 7 days to correct violation. 24 Hours .330 (A) Currently no smoke or carbon monoxide detectors inside unit. Must provide carbon and smoke detectors inside unit according to the fire department code. Call Fire Department non-emergency at (978)744-1235. Owner must send extermination report to the Salem Board of Health after the extermination has taken place. Exterminator is to return as often as needed to rid unit of pest.
