The following was discovered: _X__ 1. Operating without an exterior paint removal or abrasive blasting permit ___ 2. Failure to enclose or curtain off that area of the building undergoing abrasive blasting ___ 3. Visible emission of particulate matter or air contaminants outside of the area enclosed in accordance with provisions 6.4(b) and 6.5(a) of the regulation ___ 4. Failure to ensure adequate precautions against ground surface contamination as required by provisions 6.4(d) and 6.5(b) of the regulation ___ 5. Failure to clean property in accordance with section 7 of the regulation ___ 6. Evidence of blood lead level elevation in operator ___ 7. Other (Please describe): Exterior Paint Removal Contractor: ___ Yes __X_ No If Yes: The inspector has taken the following action: _X__ A. Ordered all exterior paint removal work to cease and desist until all violations are corrected. Owner or contractor must correct all violations indicated to continue exterior paint removal work. ___ B. Ordered all exterior paint removal work to cease and desist until all violations are corrected. Exterior paint removal work allowed to continue after all violations were corrected on site. ___ C. Exterior paint removal permit revoked because of work needing to be stopped twice on the same day due to violations. ___ D. Other (Please Describe): Repeat of any violations may result in monetary fines issued to the property owner, and the contractor (if applicable). If a contractor has been found to commit the same or similar violations at different work sites that contractor may be issued a monetary fine. Should you be aggrieved by this order, you have the right to request a hearing before the Board of Health. A request for said hearing must be received in writing in the office of the Board of Health within 7 Days of receipt of this order. At said hearing, you will be given an opportunity to be heard and to present witness and documentary evidence as to why this order should be modified or withdrawn. You may be represented by an attorney. Please also be informed that you have the right to inspect and obtain copies of all relevant inspection or investigation reports, orders, and other documentary information in the possession of this Board, and that any adverse party has the right to be present at the hearing.
