Owner must send extermination report to the Salem Board of Health after the extermination has taken place. Exterminator is to return as often as needed to rid the building of mice. Owner has 24 hours to correct violation. 5 Days .500 Kitchen cabinet is missing a drawer. Repair or replace drawer so that it's in good working condition. Repair leak and re-plaster the ceiling and repaint so that it's free from leak and in good repair. Owner has 24 hours to correct violation. 24 Hours .500 Bathroom bathtub tiles are loose and falling off the wall. Fix and repair bathroom tiles so that they are in good condition. Owner has 24 hours to correct violation. 24 Hours .500 Bathroom bathtub right side has water damage on the bottom where it meets the floor. Repair plaster so that it's in good condition. Owner has 24 hours to correct violation. 24 Hours .500 Living room wall plaster is loose and falling apart. Repair wall so that it's in good condition. Owner has 24 hours to correct violation. 24 Hours .500 I Bedroom located to the right of the front door has a broken window. Replace or repair window so that it's free of loose cracked glass and in good working condition. Owner has 24 hours to correct violation.
