I am contacting you because I have tried numerous times to contact you by phone and have not received a reply from you. A re-inspection of your apartment is required in accordance with
Chapter III, Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.00: State Sanitary Code, Chapter 1, General Administrative Procedures and 105 CMR 410.00: State Sanitary
Code, Chapter 11: Minimum Standards of Fitness for Human Habitation.
A re-inspection date of October 3, 2019 at 9:15am has been scheduled. Failure to comply and allow the Board of Health to re-inspect the unit located at 67 Tremont Street #1&2 will result
in further action by this department which includes, but is not limited to, monetary fines and a complaint being sought against you in Salem District Court.
