determined to be a(n)Oil or other combustible liquid spill.
Called received from Ken Sanderson of the DER He reported his office received a call for a spill of mineral oil at a substation on the premises.It was
unknown until further investigation if the oil contained PCBs.Further testing and cleanup will be conducted in the morning by Clean Harbors.The spill
was estimated at under 20 gallons.
DC Giunta investigated the scene,which had speedi-dry down with small booms in place and protected with a temporary fence.DC Giunta talked to
Tom Murphy from National Grid.Mr.Murphy told the DC that Connico was down to mitigate the spill earlier and would be back in the morning with a
test kit to determine if any PCBs were involved.Mr.Murphy said that he would share the test results with SFD.
Per Wally Ziehler of Footprint as reported to him from National Grid:Fast track testing determined that the spill contained less than(1)PPM of PCBs.
When the documentation is available it will be forwarded to the Salem Fire Department.The spill was also less than 10 gallons.

was determined to be a(n)Passenger vehicle fire.A Footprint Energy employee reports a truck fire at entrance gate that was put by extinguishers and
unsure is fire is completely out.The Footprint detail officer notified D69 FF Keane to investigate.The detail officer now on scene reports fire out by
Extinguishment and E1 was updated while en route.E1 on scene confirmed fire out upon arrival and also put dry chemical on battery that was
smoking.E1 cleared in service and the owner is having the motor vehicle towed from the property.The driver of the vehicle is Juan Restrepo and can
be contacted via cell 857-753-6323.

determined to be a(n)Oil or other combustible liquid spill.C3 on to investigate an oil/fuel spill of approximately 30 plus gallons onto soil during the
overnight.The MDEP notified FAO of the spill this afternoon and C3 to investigate.
On this date I was advised by Chris Bresnahan of MDEP that he received a call from a Mark Casey a consultant for one of the contractors working on
the project.He reported to the MDEP of an oil spill from a generator on the property that had leaked approximately 30 gallons into the soil.The spill
had reportedly happened the previous night but was discovered today.At this time Mr.Casey is en route to the plant and will call when he arrives.The

Fire House Narrative On 04/29/2017 at 07:35:00 dispatched To 24 Fort Avenue/Pad 45/Stack/Salem,MA 01970.The location is a Energy production plant,Other.The
incident was determined to be a(n)Chemical hazard(no spill or leak).FAO received a call direct from Northline Utilities of an odor of ammonia and no
reported injuries from chemical.I directed caller to meet apparatus at the gate and escort SFD to the odor at PAD45 near the stack.E1 on scene
investigating with Northline,TL2 to stage out front per C5 now on scene.C5 now reports ammonia was being pumped out of a 55 gallon drum for the
boiler and no readings are on the meter.C5 reports drum has been capped and E1 and C5 are clear in service.

The following actions were performed on scene:Arrived at the gate to 24 Fort Ave and was met by Northline Safety personnel who escorted C-5 and
E-1 to an area were ammonia was detected by smell in their work area. T-2 was directed to stage at the main gate.Some of the work areas were being
evacuated by the construction workers.Upon investigation of the area it was determined that the odor was coming from a 55 gallon drum that was
open and being pumped to a process to treat the water for the boilers.The hose was removed from the drum and the cap was secured back on the
drum.Safety personnel from O'Connor construction used a meter capable of detecting ammonia to sample the area and had no readings.The
company working on the ammonia process(subcontractor for O'Connor construction)stated they would move the 2 full and 4 empty 55 gallon drums
to an area away from were other personnel are working.While moving the pallet that contained 2 full drums and 2 empty drums by a lull fork lift the
drums fell off the pallet due to not being secured properly.Upon investigation of the drums there was no damage visible.
All areas of the work site were not evacuated due to the evacuation message going out only over the radio and not all construction personnel carry
radios.There is a public address system on site and I was informed by someone from Iberdrola safety that the only person who knew how to operate
to the public address system no longer worked for the company.Iberdrola construction needs to review the site notification process so all safety
personnel and site forman are informed of emergencies on the site.This will help with controlling miss information on site and personnel will know
whether to evacuate or continue working.

determined to be a(n)Chemical spill or leak.FAO Larrabee received a call direct via UHF radio from Lt.Brophy on the power plant detail of an
Ammonia leak and requested E1 and C5 to respond.CS was notified and responded with E1 only.Lt Brophy and Lt.Cook both reports building is
evacuated.C5 on scene locates the building and reports a valve was left open while treating water with a low concentration of ammonia with no
readings on the meter.C5 is releasing E1 and standing by with workman and all of the details at the power plant. C5 reports at 1400hrs all workers
returned back into the building and C5 standing by speaking to personal at the power plant.C5 cleared in service.
13:14:00 arrived on scene.
The following involvements were noted:Arrived and directed by fire personnel on site to where there was an odor of ammonia,construction
personnel were evacuating the building,C5 made contacted with safety personnel and determined there was a valve that was opened during a
flushing process,the valve was secured by workers,several gallons of water with a low concentration of ammonia had leaked on to the floor,safety
officer with ammonia meter had no readings,workers were allowed back into building to work,C5 remained on scene with Jon Sodomin of Iberdrola
safety to learn about the flushing process and discuss accountability,

A call was received from the premises reporting a gas leak in the turbine building.The detail on scene was contacted and was already aware of the
incident and advised they were investigating it.Lt.Bresnahan determined no further response was needed.They check along with the Northline
Safety person and no readings were detected.All was ok.
