Subject: Pest Management Protocol for 13 Jackson Terr
Dear: Salem Board OF Health,
I am writing to let you know what was done at 13 Jackson Terrace and what will be
done to address the problem as you requested. 1-According to Assurance Pest
Solution: They will seal off entry points along the exterior foundation, any existing
entry point at appliances and setup mouse bait stations and trapping device. The
main service areas will be the basement, attic, garage, and kitchen. 3 weeks follow
to check and refresh all mice equipment, remove mice caught if any, and assess
activity levels remaining. Long-lasting material such as: Xcluder stainless steel
rodent proof mesh, 1/4 inch metal wire screening, exterior sealant, and up to l0lbs
of cement, will be used by Assurance Pest Solution to do the job. The job is
warranties for 6 months. 2-Tenants were intruded to keep their trash cans close all
time, put extra trash bags on top of the trash cans the day of collection, keep the
area clean of all food debris, keep sink clear of dirty dishes 3-After the next 6-
month Assurance Pest Solution will be servicing the property every 3 month to
avoid fiu-ther infestation

Equipment Summary
Technician Comments:
Today we performed the mouse service.Upon inspection we found mild activity on the second unit,prior history of mouse entry ofthe third and were denied entry of
unit one.We set up equipment in the half side basement,unit 3 and 2 as well as set up exterior equipment to cull the discovered rat activity along with keeping
pressure off of the home.A few structural repairs are needed and have been logged into the account documents.
The bulkhead has a large structural gap as seen in the photo,the weep hole where wires are being ran on the left side of the front porch may need screen.
Upon the follow up service we will assess the condition of current rodents however we suggest an exterior maintenance plan to keep the local rodent population
from clustering around the home and attempting entry.The current state of the property and repairs needed are required to assure rodent exclusion and hold the
warranty to the property.
The initial mouse service was performed today.The interior and exterior entry points that were found were sealed with xcluder material.The interior areas had a
baiting and trapping system installed.

The 3 week follow up service is where you might see mice in different areas of the home.If you do find a mouse caught in a trap,you can dispose of this in the trash.
Let us know if you have caught and disposed of any mice when we arrive.This service checks all the mouse equipment and exclusion work.We will replenish the
baits,remove and replace the traps and seal any new entry points found.
The mouse service performed will not work if these conducive conditions are not met with by the homeowner/tenant. 1.Keeping food stored in pest proof containers.
2.Keeping all areas clean of all food debris and keeping the sink clear of dirty dishes.3.Cleaning the grease from the stove area,especially the sides.4.Garbage

Dear Property Owner:
This office has received a complaint regarding rodents on your property located at 13 Jackson Terrace in the city
of Salem,.Massachusetts.
An on-site inspection was conducted on December 6,2023 by Jeffrey Barosy,Sanitarian for the Board of Health.
During the inspection rat burrows at the rear of the house and holes in garbage and recycling bins made by rats were
observed on the property.
Raccoons,rodents,etc. are considered a neighborhood nuisance and community health hazard.
All parts of the property rats are harboring must be exterminated inside and out by a licensed exterminator. After
extermination has taken place any burrows or holes must be filled and monitored and you must take all necessary
steps to prevent any infestation from occurring in the future. Additionally,you must submit to the Salem Board of
Health an Integrated Pest Management protocol for this property stating what actions have been taken and what
actions will be taken to prevent this infestation from occurring in the future.
In accordance with Mass General Laws,Chapter 111,Section 123 you are ordered to retain the services of a
licensed exterminator and suppress any rodent population within fourteen(14)days of receipt of this order
and a copy of this invoice and a treatment plan must be forwarded to this department upon completion of the
extermination.
Should you be aggrieved by this order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 Days of receipt of this
order. At said hearing,you will be given an opportunity to be heard and to present witness and documentary
evidence as to why this order should be modified or withdrawn. You may be represented by an attorney. Please
also be informed that you have the right to inspect and obtain copies of all relevant inspection or investigation
reports,orders,and other documentary information in the possession of this Board,and that any adverse party has
the right to be present at the hearing.
