(978) 741-1800 DOMINICK PANGALLO healthsalem.com DAVID GREENBAUM,RS MAYOR HEALTH AGENT September 21, 2023 255 Jefferson Avenue, LLC Attn: Avi Golani 32 Van Roosen Road Newton, MA 02459 Dear Mr. Golani: In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum Standards of Fitness for Human Habitation, a recent re-inspection was conducted of your property located at 255 Jefferson Avenue Unit IF, conducted by Jeffrey Barosy, Sanitarian of the Salem Board of Health on September 18, 2023. At that time the following items were still outstanding: Item # 1: Both the bathroom ceiling tiles, and the pantry ceiling still show signs of water leakage. Item #2: The bathtub wall beneath the shower head is still loose and separating from the bathroom wall. Item #3: The hot water temperature at the sinks is still at 141°F. Item #4: The inspector has received no confirmation of extermination of mice in the dwelling unit. Item # 5: Occupant stated gnats/sewer flies were still present, but the inspector could not confirm at time of re-inspection. Item #6: This bedroom window's child safety lock is still broken. During the re-inspection a new violation was observed: 105 CMR 410.230: The bathtub's lining is coming loose. Repair bathtub so its surface is intact with a smooth, easy to clean service. The owner has 30 Days to correct this violation. Please correct the outstanding violations and contact this office to schedule a final re-inspection of this apartment. Except for the new violation, the owner has 14 days from receipt of this letter to correct violations. Failure to correct violations will result in this matter being brought to housing court.
