Dear Sir/Madam:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum
Standards of Fitness for Human Habitation, a recent re-inspection was conducted of your
property located at 2 Pioneer Terrace Unit E, conducted by Jeffrey Barosy, Sanitarian of the
Salem Board of Health on July 24, 2023. At that time the following items were still outstanding:
Item# 1: Mice droppings are still present and the hole in the wall in the bedroom near
the window has not been repaired. Continue to exterminate and repair the hole in the
bedroom wall.
Please correct the outstanding violations and contact this office to schedule a final re-inspection
of this apartment.

Tenant from 2e wasn't home to speak with.I left a note for tenant to call when she removes items from closets and book case.I 1,'
need to get to all baseboards,For thorough inspection.
Found most of the 9 bait stations empty.
I put down 12 bait stations and added 6 plastic snap traps.
I suspect mice are coming from 2a,a vacant unit below.Found nests in walls of 2a.
Units on same side of building,as 2e,are units 2a,2c,29.,and they all have some level of infestation.Units in the opposite side`1
of the building show no activity.
Basement shows no activity. -

Tenant called and I met with her. -
She explained a friend found holes,one where base cabinet meets wall to right of stove,and one under space between heater ,
and floor. _
There are no passages possible in either spaces,due to construction of building.
I put Stufflt in areas of concern to make tenant comfortable.
Tenant also explained,friend has removed all items from closets,prior to my requests,and couldn't find passages during his--Y'
own inspection.Tenant explained,prior to my requests,the inspector from board of health looked behind book case with
flashlight and assured her no activity is seen behind book case.
Checking units 2C,and 2g I barricaded several newly formed passages around piping under sinks.
I used 50 final soft baits.
I installed new door sweeps on entry doors in units 2a,2b and 2g.
Friend of tenant in 2e had installed door sweeps for front and rear doors.
I explained to tenant of 2e,I'll be back in 2 weeks for follow up.
