Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Violations? Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 24 Hours .500 1. The shower stall in the bathroom has chipping and peeling plaster on .630 (12) its ceiling and brown discoloration indicative of a water leak. Find the source of the water leak and repair it. Remove chipping, peeling and discolored plaster and re-plaster affected area. .235 2. The shower stall's drain cover is disconnected from the drain. Repair 30 Days the drain cover so it is properly affixed to drain. .500 3. The kitchen ceiling above the kitchen sink has a hole from a water 24 Hours .630 (12) leak. Find the source of the water leak and repair it. Repair the hole in the kitchen ceiling so the kitchen ceiling is restored to the condition it was in before the water leak. .100 4. The caulk for the kitchen sink counter where the counter meets the 30 Days wall has deteriorated. Re-caulk the area between the kitchen sink counter and the wall. .235 5. The kitchen sink drainage pipe is taped to stop a water leak. Repair 24 Hours .630 (13) the kitchen sink's drainage pipe so it does not leak and no longer needs to be taped to stop water leakage. - .500 6. Doorway leading to rear common hallway has a doorjamb that is 3' 0 Days loose and cracking lengthwise. Repair this doorway's doorjamb so it J is intact and no longer loose. .550 7. Mice droppings were found on floor in the kitchen beneath the kitchen 24 Hours .630 (17) window on the left. The owner is to exterminate mice in the dwelling unit and all adjacent walls. Any holes leading into the dwelling unit must be I)lu�;ged to ;prevent re-entry from mice. Extermination is to continue as needed until mice no longer present a public health nuisance in the dwelling unit. .330 8. The bedroom adjacent to the bathroom has a missing smoke 24 Hours .630 (15) detector. Provide a properly working smoke detector for this bedroom. .500 9. The bedroom adjacent to the bathroom has holes in the walls and. 30 Days floor around the baseboard heater indicative of mice. Repair the holes in the walls and the floor and seal off gaps to preve t mice entry. .500 10. The bedroom adjacent to the bathroom has a crack in the doorway's 30 Days doorjamb. Repair the crack in this doorway's doorjamb. N/A 11. NOTE: Occupants stated there is a rotten egg odor coming from the N/A hot water at the kitchen and bathroom sinks. Inspector did not observe this odor at the time of inspection. Inspector advises the owner to investigate the source of this odor and correct it. N/A 12. The owner has 30 DAYS to correct all violations listed in this report. N/A The owner must start making good-faith effort (reporting to the Salem Board of Health what actions will be taken in writing, hiring contractors that may be needed, or starting necessary work on his/her own)within the time frame noted beside each violation. If an extension beyond 30 days is needed to make corrections, Owner must request a hearing before the Board of Health within 7 DAYS of receipt of this report and acquire approval for the extension.
