In accordance with Chapter 111,Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000;
State Sanitary Code, Chapter 1: General Administrative Procedures and 105 CMR 410.000: State Sanitary Code,
Chapter Il: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 32
Ward Street Unit 1 F permitted by occupant Kelsey Stiefel conducted by Jeffrey Barosy,Sanitarian on July 24,
2023 at 9:00am.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies
available to them as outlined in the enclosed inspection report form.

Specified Time Reg. #410. Violation(s)
Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410,000. Upon
Inspection the following were noted:
30 Days .220 1. The bathroom's exhaust fan does not work. Repair the bathroom's
exhaust fan so it exhausts moisture from the bathroom.
.235 2. The bathroom sink is coming loose from the wall. Repair the
30 Days bathroom's sink so it is properly affixed to the wall.
.110 3. Several of the bathtub's wall tiles have come loose and fallen off the
30 Days wall. Repair the bathtub's wall so wall tiles are properly affixed and
have smooth and easily cleanable surfaces.
.500 4. Ceiling tiles in the bathroom and in the kitchen above the kitchen sink
24 Hours .630 (12) show signs of water leakage with brown discoloration and sagging.
Find the source of the water leak and repair it. Replace affected
ceiling tiles with new ceiling tiles.
.235 5. The kitchen's baseboard heaters' cover is falling off. Repair the
30 Days baseboard heaters' cover so it is properly mounted on the baseboard
heater. _
.220 6. The kitchen ceiling's exhaust vent does not work. Repair kitchen
30 Days ceiling's exhaust vent so it turns on and off and exhausts air as
designed.
.235 7. The refrigerator door is loose with a gap in the gasket near the bottom
30 Days and it is held closed with a child safety strap. Repair refrigerator door
so it no longer has a gap in its gaskets, and it can stay closed under
its own p9wer.
.540 8. The kitchen window on the right has a hole in its screen. Repair the
30 Days hole in this window's screen.
500 9. The kitchen window on the left has broken glass in its bottom sash
24 Hours T. *
630 (12) and it is currently covered up with insulation. Repair the glass in this
_ window's bottom sash.
.550 10. Mice droppings were found behind the stove and a dead mouse was
24 Hours .630 (17) found in the basement. The owner is to exterminate mice in the
dwelling unit and all adjacent walls. Any holes leading into the
dwelling unit must be plugged to prevent re-entry from mice.
Extermination is to continue as needed until mice no longer
_ resent a public health nuisance in the dwelling unit.
.235 11. The bedroom on the far left when standing in the kitchen has a
30 Days baseboard heater with broken covers under the window on the far
left. Repair this baseboard heater's covers so they are properly
mounted on to the baseboard heater.
300 12. The front common hallway has a light on the wall above door to
[30 Days basement that flickers when turned on. Repair light so it no longer
flickers when turned on and p rovides ro er illumination.
.500 13. In the front common hallway, the doorknob for the door to the
[30 Days basement is broken and warped. Repair this doorknob so it is
properly affixed to the door and functions as designed.
.500 14. In the basement one of the boilers has a water leak. Repair this boiler
30 Days so it no longer leaks water.
.300 15. In the basement the laundry room has a ceiling light that does not
30 Days work. Repair this ceiling light so it provides proper illumination to this
area of the basement.
.550 16.A hole for a rat burrow was found outside at the side of the property
24 Hours .630 (17) facing 30 Ward Street. The owner is to exterminate rats on the
property. Any rat burrows on the property must be filled in.
Extermination is to continue as needed until rats no longer
resent a public health nuisance on the property.
.560 17. There are remnants of a melted plastic appliance hanging down from
30 Days the bottom of the second-floor deck. Have melted plastic removed
from deck and discarded in a sanitaral manner.
N/A 18. NOTE: Occupant informed inspector there are out of use garbage
N/A and recycling bins on the property. If these bins are out of use have
them removed from the property_.
N/A 19. NOTE: Occupant informed inspector that the door leading from her
N/A kitchen into the front common hallway swells when it is humid,
preventing the door from closing. The inspector did not observe this
at the time of inspection. Inspector advises owner to have this door
repaired to it can open and close with ease in any weather.
N/A 20. NOTE: Occupant informed inspector electricity stops working from
N/A time to time when she tries to use her appliances. The inspector is
not an electrician, and he did not witness this at the time of
inspection. Inspector has referred this to the City's Electrical
Department
N/A 21. NOTE: Occupant informed inspector her heating system does not
N/A work. The inspector could not observe this at the time of inspection of
the high temperatures of the summer season. Inspector advises the
owner to have heating system for this unit working before the
beginnin�j of the Heating Season which begins September 151n
N/A 22. Owner has 30 DAYS to correct all violations listed in this report.
N/A Owner must start making good-faith effort (reporting to the Salem
Board of Health what actions will be taken in writing, hiring
contractors that may be needed, or starting necessary work on
his/her own) within the time frame noted beside each violation. If an
extension beyond 30 days is needed to make corrections, Owner
must request a hearing before the Board of Health within 7 DAYS of
receipt of this report and acquire approval for the extension.
One or more of the above violations may endanger or materially impair the health, safety and well-being of the
occupant(s)
