Chapter II: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 93
Ocean Avenue Unit 2 permitted by occupants Sarina Saindon &Samantha Scalfani conducted by Jeffrey
Barosy,Sanitarian on July 18,2023 at 9:15am.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is.the property owner's.
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies
available to them as outlined in the enclosed inspection report form.

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
30 Days .500 1. The bedroom off the study has a window on the left with a bottom
sash that is difficult to open and close. The same window has a
screen that does not close completely. Repair this window so its
sashes and screens o en and close with ease.
.500 2. The bedroom off the living room, next to the kitchen, has a hole in the
30 Days ceiling next to ceiling light. Repair the hole in this ceiling.
.235 3. The bedroom off the living room, next to the kitchen, has ceiling lights
30 Days that do not work when the light switch is activated. Repair this
bedroom's ceiling lights so they turn on and off when light switch is
activated.
.500 4. The bedroom off the living room, next to the kitchen, has a closet with
24 Hours .630 (12) a ceiling that shows signs of water damage with chipping and
discolored plaster. Find the source of the water and repair. Replaster
_ effected portions of this closet's ceiling. -
.540 5. The bedroom off the living room, next to the kitchen, has a window
30 Days with a torn screen. Repair the torn screen.
.110 6. The small shelf with ridges in bathtub has a crack on its top surface.
30 Days Repair the surface of this shelf so it is free of cracks and has smooth,
easily cleanable surfaces.
.150 7. Hot water at bathroom and kitchen sinks peaks at 153°F. Lower hot
30 Days water temperature so it is between 110°F and 130°F.
.500 8. The handle for the casement window above kitchen sink is broken.
30 Days Repair this window's handle so it can be opened and closed with
ease.
.235 9. Garbage disposal's inlet pipe is broken. Repair garbage disposal's
24 Hours 630 (13) inlet pipe.
.330 10. The smoke detector in the living room does not work. Repair or
24 Hours .630 (15) replace smoke detector so it works properly.
.500 11. The study has a window on the right with a broken handle on the
30 Days bottom sash. Repair the handle on this window's bottom sash.
.500 12. The exterior front stairway is propped up by a piece of a cinder block.
24 Hours .630 (12) Repair the exterior front stairs so they are properly supported.
N/A 13. Owner has 30 DAYS to correct all violations listed in this report.
N/A Owner must start making good-faith effort(reporting to the Salem
Board of Health what actions will be taken in writing, hiring
contractors that may be needed, or starting necessary work on
his/her own)within the time frame noted beside each. If an extension
beyond 30 days is needed to make corrections, Owner must request
a hearing before the Board of Health within 7 DAYS of receipt of this
report and acquire approval for the extension.
