In accordance with Chapter III, Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.00;State Sanitary Code, Chapter
1: General Administrative Procedures and 105 CMR 410.00: State Sanitary Code, Chapter 11: Minimum Standards of Fitness for Human
Habitation, an inspection was conducted of the property 65 Boston Street#8,conducted by Elizabeth Gagakis, Senior Sanitarian on
January 24, 2020 at 9:00am.
Notice:if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of
.lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention
and Control. For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive
action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in
Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request for said hearing must be
received in writing in the office of the Board of Health within 7 days of receipt of this Order. At said hearing,you will be given an opportunity to
be heard and to present witness and documentary evidence as to why this Order should be modified or withdrawn.You may be represented by
an attorney. Please also be informed that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,
orders and other documentary information in the possession of this Board, and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies available to them as
outlined in the enclosed inspection report form.

Based upon a tenant complaint a permitted inspection was conducted in accordance with
Article II of the State Sanitary Code, 105 CMR 410.000. Upon inspection, the following
were noted:
24-48 hours .550 1. Tenant had complaints of rodent activity.At the time of inspection, mouse
droppings were observed in the closet opposite the bathroom and in the closet in
the first bedroom. Owner to hire services of licensed exterminator to inspect and
treat the unit and entire property; treatments must continue until problems are
controlled. Any and all recommendations by the exterminator of repairs to prevent
entrance of mice must be completed. Please forward invoices for service to the
Board of Health for our records. Owner to correct this violation within 24-48 hours.
7 days .500 2. There is a hole in the floor at the front left corner of the fireplace. Seal this hole to
prevent the entrance of pests. Owner to correct this violation within 7 days.
7 days .500 3. There is a gap around the pipe under the kitchen sink.Seal around the pipe so the
gap is eliminated and will prevent the entrance of pests. Owner to correct this
violation within 7 days.
7 days .500 4. There is a gap around a pipe exiting the cabinet beneath the sink. Seal around the
pipe so the gap is eliminated and will prevent the entrance of pests. Owner to
correct this violation within 7 days.
7 days .500 5. Crack behind refrigerator has been repaired but is not fully sealed. Ensure entire
crack behind the fridge is sealed to prevent the entrance of pests. Owner to correct
this violation within 7 days.
7 days .500 6. There are gaps underneath the right end of the kitchen baseboard heating unit
around the pipes. Ensure all gaps around pipes are sealed to prevent entrance of
pests. Owner to correct this violation within 7 days.
24-48 hours .602 7 There is a collection of items and broken barrels piled on the side of the building
outside tenant's back exit. All trash must be kept in barrels with tightly-fitting lids
and any debris and items around the trash area must be removed; trash area must
be maintained in a clean, sanitary manner free from trash and debris. Owner to
correct this violation within 24-48 hours.
7 days .500 8. There are gaps underneath the front hall baseboard heating unit around the pipes.
Ensure all gaps around pipes are sealed to prevent entrance of pests. Owner to
correct this violation within 7 days.
24-48 hours .482 9. The smoke detector in the upstairs hallway is hanging from the ceiling by the wires.
Screw smoke detector into the bracket in the ceiling so it is positioned properly.
Owner to correct this violation within 24-48 hours.
7 days .500 10. There are gaps underneath the baseboard heating unit around the pipes in both
bedrooms. Ensure all gaps around pipes are sealed to prevent entrance of pests.
Owner to correct this violation within 7 days.
7 days .500 11. There is a stain inside the corner of the ceiling in the closet of the second bedroom.
Investigate for leaks and repair any sources of leaking found; scrape and repaint
interior of closet so ceiling and walls are intact and in good condition. Owner to
correct this violation within 7 days.
7 days .500 12. The sixth stair up from the front hall is soft when stepped on. Repair the stair so it
is stable and in good condition. Owner to correct this violation within 7 days.
7 days .253 13. The exterior lights at the front entry to the unit do not work. Owner must provide
light fixtures in good working order so that illumination may be provided for the safe
&reasonable use of every exterior stairway. Owner to correct this violation within 7
days.
One or more of the above violations may endanger or materially impair the health, safety and well-being of the occupant(s)
