You are hereby ORDERED to make a gdod-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Violation(s) Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon inspection, the following were noted: 7 days .354 1. Tenant had complaint regarding metering of the gas for hot water. At the time of inspection, it was observed and confirmed by National Grid that only one hot water tank existed for the building, serving all units, and is metered to unit 1. Owner shall provide the electricity and gas used in each unit unless each unit is served by meters serving only that unit, or unless a written agreement provides for payment by the occupant. Owner to hire a licensed plumber to remove hot water tank from unit 1's gas mater and place it onto a common meter. Please provide invoices for work to the Board of Health for our records. Owner to correct this violation within 7 days. 7 days .500 2. There is evidence of a leak in the living room, as indicated by stains and cracks on the ceiling. Owner to investigate for source of leaking and make any repairs necessary; scrape and repaint living room ceiling so it is intact and in good condition. Owner to correct this violation within 7 days. 24-48 hours 482 3. There is a smoke detector bracket in the kitchen missing a smoke detector. Provide a smoke detector in this bracket; consult with fire marshal for questions concerning placement. Owner to correct this violation within 24-48 hours, as indicated when owner was spoken with onsite at the time of inspection. All smoke detector violations forwarded to fire prevention for follow up. 24-48 hours .482 4. Smoke detector is missing from ceiling in office off kitchen. Provide a smoke detector in this location. Owner to correct this violation within 24-48 hours, as indicated when owner was spoken with onsite at the time of inspection. All smoke detector violations forwarded to fire prevention for follow up. 7 days .500 5. There are screws sticking out of the wall in a corner of the second bedroom. Repair wall from the outside so that screws are not sticking into the room from the outside and the interior walls are in good condition. Owner to correct this violation within 7 days. 24-48 hours .482 6. There did not appear to be any carbon monoxide detectors in the unit. CO detectors must be installed within ten feet of every sleeping area. Owner to correct this violation within 24-48 hours, as indicated when owner was spoken with onsite at the time of inspection. All smoke detector violations forwarded to fire prevention for follow up. 7 days .500 7. There is evidence of a leak in the master bedroom, indicated by stains on the ceiling towards the front window. Owner to investigate for source of leaking and make any repairs necessary; scrape and repaint the ceiling so it is intact and in good condition. Owner to correct this violation within 7 days. 24-48 hours .482 8. Smoke detector is missing from ceiling in master bedroom. Provide a smoke detector in this location. At the time of inspection, it was observed and confirmed by National Grid that the hallway light at the side entry door used by more than one tenant is metered to unit 1. Owner to hire services of a licensed electrician to remove this and any other commons lights from unit 1's meter and place onto a house meter. Please provide invoices for work to the Board of Health for our records. Owner to correct this violation within 7 days.
