In accordance with Chapter III, Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.00;State Sanitary Code, Chapter
1: General Administrative Procedures and 105 CMR 410.00:State Sanitary Code, Chapter 11: Minimum Standards of Fitness for Human
Habitation, an inspection was conducted of the property 14 Mount Vernon Street#2 permitted by occupant Kelly Hill,conducted by
Elizabeth Gagakis, Senior Sanitarian on May 18,2018 @ 10:00 a.m.
Notice: if this rental unit is occupied by a child or children under the age of 6 years,it is the property owner's responsibility to notify tenants of
lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention
and Control. For further information or to request an inspection, contact the Salem Board.of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive
action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in
Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request for said hearing must be
received in writing in the office of the Board of Health within 7 days of receipt of this Order. At said hearing,you will be given an opportunity to
be heard and to present witness and documentary evidence as to why this Order should be modified or withdrawn.You may be represented by
an attorney. Please also be informed that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,
orders and other documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies available to them as

Based upon a tenant complaint a permitted inspection was conducted in accordance with
Article II of the State Sanitary Code, 105 CMR 410.000. Upon inspection, the following
were noted:
7 days .354 1. Light in ceiling in third floor child's bedroom is hanging from ceiling and missing a
cover. Secure light and provide a cover so it is in good condition. Owner to correct
this violation within 7 days.
7 days t .500 2. There are some holes in the wall near the door in third floor child's bedroom.
Repair wall so it is intact and in good condition. Owner to correct this violation
within 7 days.
7 days .500 3. There is some staining in the corner of the skylight window in the third floor guest
room and some stains below the window, indicating a potential leak. Investigate for
any source of leaks and repair so they do not reoccur; scrape and repaint stained
areas so they are intact and in good condition. Owner to correct this violation within
7 days.
7 days .354 4. Ceiling light in third floor guest room is missing a cover. Provide a cover for this
light. Owner to correct this violation within 7 days.
7 days .500 5. There is some cracked plaster and stains on the ceiling near the closet area in the
small dining area of the third floor. Investigate for any source of leaks and repair so
they do not reoccur; scrape and repaint stained areas and repair plaster so the
area is intact and in good condition. Owner to correct this violation within 7 days.
days .500 6. There is a hole in the wall in the stairway up to the third floor. Repair wall so it is
intact and in good condition. Owner to correct this violation within 7 days.

7 days .354 7. Faucet at sink in the kitchenette area on third floor is missing an aerator. Provide
an aerator for the faucet. Owner to correct this violation within 7 days.

7 days .500 8. There is peeling paint on the ceiling and around the window in the master bedroom
on the third floor. Scrape and repaint any affected areas so walls/ceiling are intact
and in good condition. Owner to correct this violation within 7 days.
7 days .500 9. There is cracked plaster at the door up to the third floor. Repair plaster so walls are
in good condition. Owner to correct this violation within 7 days.
7 days .500 10. The porch door in back bedroom on the second floor has several panes of glass
missing putty. Putty around all panes of glass in the door so they are properly
installed and secure. Owner to correct this violation within 7 days.
24-48 hours .602 11. There are totes with standing water in the back yard and the remnants of an old
pool in a corner of the yard. Standing water must be removed to prevent mosquito
breeding and all debris must be cleaned up; property must be maintained in a
clean, sanitary manner free from any trash or debris. Owner and tenant to correct
these violations within 24-48 hours.
