We have a building permit application into the building department and working through the checklist to secure a building permit. Here is the information for the conditions set by the Board of Health: 1. Project Manager: Mark Livesey Mark@Symesassociates.com Cell (978) 836-8127. 2. No DEP tracing number issued. New units to be constructed within the footprint of the existing building. 3. Attached is a copy of the licensed Asbestos Inspector’s Report. 4. DEP filing not required Caution: This email originated from outside our system. Please do not click links or open attachments unless you recognize the sender and know the content is safe. 2 5. Original Drainage plan was completed per original development, certificate of compliance issued 2022. No changes. 6. Inspection report from A1 Exterminators is attached 7. Rodent Control plan from A1 Exterminators is attached 8. Dust Control during demolition will be provided by Salem Fire Department detail. Street sweeping will be provided by Meninno Construction Co. – site contractor. 9. Kidder Wrecking Inc. will dispose of all demolition materials as they are generated. Meninno Construction Co. will dispose of any extra materials during excavation of new footings. 10. Trash and recycling contract is attached. 11. All trash and recycling containers are located within chain-link fence enclosures with privacy slats 12. Plan approved by Salem Fire for access 13. Noise levels from the resultant establishment generated by operations will not increase the broadband sound level by more than 10 dB(A) above the ambient levels measured at the property line. 14. All dumpster trash areas are currently enclosed. Temporary construction dumpster will be enclosed within the construction fence during the project. 15. The Developer will disclose in writing to the Health Agent the origin of any fill material needed for the project. 16. All waste materials shall be disposed of in an environmentally sound manner as described by the Board of Health 17. The Developer will notify the Health Agent when the project is complete for final inspection and confirmation that the above conditions have been met. Look forward to getting this project started. Finishing up the 1st new building – 136 Canal St (22 apartments and 5 retail spaces) and we are going for a partial certificate of occupancy for this building soon. Trying to occupy 9/15. I am reaching out to see if there is anything you need from me at this point in the project. Planning Board requires us to notify the Board of Health upon completion of the project for compliance, which we anticipate early 2021 – but wanted to see if you can confirm compliance for 136 Canal St now to line up with our partial certificate of occupancy. Now we are ready to move forward with the new buildings. The Planning Board decision lists a number of requirements (below) that we need to provide for the Board of Health and I wanted to get started on getting this information over to the Board of Health now, as we will be applying for a building permit over the next couple of weeks. Are you the best contact to send this information? Thanks, Mark 1. Board of Health a. The owner shall comply with the following specific conditions issued by the Board of Health: b. The individual presenting the plan to the Board of Health must notify the Health Agent of the name, address, and telephone number of the project (site) manager who will be on site and directly responsible for the construction of the project. c. If a DEP tracking number is issued for this site under the Massachusetts Contingency Plan, no structure shall be constructed until the Licensed Site Professional responsible for the site confirms that the site meets the DEP standards for the proposed use. d. A copy of the Licensed Asbestos Inspector’s Report must be sent to the Health Agent. e. A copy of the Demolition Notice sent to the DEP, Form BWPAO6, must be sent to the Health Agent. f. The developer shall give the Health Agent a copy of the 21E report. g. The developer shall employ a licensed pesticide applicator to exterminate the area prior to construction, demolition, and/or blasting and shall send a copy of the exterminator’s exterminator’s survey report, treatment plan and treatment reports to the Health Agent. h. The developer shall maintain the area free from rodents throughout construction. i. The developer shall submit to the Health Agent a written plan for dust control and street sweeping which will occur during construction. j. The developer shall submit to the Health Agent a written plan for containment and removal of debris, vegetative waste, and unacceptable excavation material generated during demolition and/or construction. k. Noise levels from the resultant establishment(s) generated by operations, including but not limited to refrigeration and heating, shall not increase the broadband sound level by more than 10 dB(A) above the ambient levels measured at the nearest abutting residential property line. l. The developer shall disclose in writing to the Health Agent the origin of any fill material needed for the project. 9 m. The resultant establishment shall dispose of all waste materials resulting from its operation in an environmentally sound manner as described to the Board of health. n. The drainage system for this project must be reviewed and approved by the Northeast Mosquito Control and Wetlands Management District. o. The developer shall notify the Health Agent when the project is complete for final inspection and confirmation that above conditions have been met. Mark J.
