Dear Ms. Klosowski,
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum
Standards of Fitness for Human Habitation, a recent re-inspection was conducted of your
property located at 11 Summer Street Unit 1 R, conducted by Jeffrey Barosy, Sanitarian of
the Salem Board of Health on May 1, 2023. At that time the following items, based on the
February 17, 2023 inspection report, were still outstanding:
Item #5: Kitchen window on right still has a broken string for the front sash's
counterbalance on the right.
Item #6: Kitchen window on the left still has two sashes that do not align properly,
preventing them from locking correctly.
Item #7: The bedroom adjacent to the kitchen still has a hole in the floor around the
vertical pipe in the corner of the bedroom to the right of the door when walking into the
bedroom. Provide a properly fitting escutcheon for this pipe.
Item #8: The ceiling light is in the process of being replaced with a new ceiling light.
Occupant informed inspector the new ceiling light was on the property but needs to be
installed.
Item #9: Pantry window still has sashes that do not align properly, preventing the
window from locking properly.
Item #10: Pantry floor in front of sink is still warped and deteriorating.
Item #11: Mice traps were observed in dwelling unit. Inspector will follow up with
property owner on status of mice extermination.
Item #14: Living room's window sashes do not stay up under their own power and still
need to be propped open.
Item #15: Living room's middle window's top sash is still screwed shut.
Item #17: The portion of the ceiling that fell away has been replaced with new drywall.
However, there are still areas along the edge of the new drywall that are exposed and
there is a part of the ceiling near the vertical pipe that is uncovered. Cover the exposed
part of the ceiling and seal the edges around the drywall.
Item #19: Bedroom adjacent to living room still has brown discoloration on ceiling above
entryway into living room.
Item #21: Bedroom adjacent to living room has a stand-alone window with misaligned
sashes preventing the window from locking properly.
Item #22: Bedroom adjacent to living room has a group of windows across from
entryway into living room. The middle window has a bottom sash that does not stay
open under its own power.
Please correct the outstanding violations and contact this office to schedule a final re-inspection
of this apartment.
All other violations noted in the inspection report of February 17, 2023 have been
corrected.
Thank you for your cooperation in this matter.
