Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection,contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Violation(s) Based upon a tenant complaint a permitted inspection was conducted in accordance with Article 11 of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 24 Hours .550 1. Mice droppings were found on floor in bathroom and on kitchen .750 (0) countertop next to stove Owner is to exterminate mice in the dwelling unit and all adjacent walls. Any holes leading into the dwelling unit must be plugged to prevent re-entry from mice. Extermination is to continue as needed until mice no longer present a public health nuisance in the dwelling unit. Owner has 24 HOURS to start making good-faith effort to correct violation. 7 Days .551 2. Windows throughout the dwelling unit have either missing screens or torn .553 screens. Repair torn screens. Provide screens for windows with missing screens. Window screens are required to be installed between April 1st and October 30th. Owner has 7 DAYS to start making a good-faith effort to correct violation. 7 Days .280 3. Bathroom lacks a vent to the outside and lacks a window that opens to the outside. Install either a vent to the outside or a window to the outside in bathroom. Owner has 7 DAYS to start making a good-faith effort to correct violation. 24 Hours .500 4. Bathroom ceiling above sink shows evidence of water leakage with discolored ceiling tile. Find source of water leakage and repair. Replace ceiling tile. Owner has 24 HOURS to start making a good-faith effort to correct violation. 7 Days .190 7. Hot water at kitchen and bathroom sinks peaks at 55*F. Restore hot .750 (A) water to dwelling unit so it is between 110*F and 130*F. Owner has 7 DAYS to start making agood-faith effort to correct violation. _ 24 Hours .201 8. Temperature in dwelling unit at time of inspection was 63*F. The .750 (B) thermostat was set for 70*F. The inspector felt no heat coming out of the baseboard heaters. Repair heating facilities so that during the Heating Season temperatures are between 68*F and 78*F between 7:00am and 11:00pm, and between 64*F and 78*F between 11:01 pm and 6:59pm.
